Prepare all the necessary documents listed below before you deliver the application, otherwisetheapplicationmightberefusedbytheEmbassy/ConsulateGeneraloftheRepublic ofPoland.TherequireddocumentshavetobesubmittedinPolishorEnglishversion. Whilethevisaapplicationisbeingprocessed,theapplicant'spassportremainsat theEmbassy/ConsulateGeneraloftheRepublicofPoland THEAPPLICATIONMATERIALSHOULDBEPLACEDINTHEFOLLOWINGORDER

  3. REQUIREDDOCUMENTS VISAAPPLICATION Visaapplicationformwithoutblanks,signedbytheapplicant-ifminor (below18yearsold),signedbybothparentsorlegalguardians; thesignatureastheoneinthepassport. Acolorphotographywiththefollowingspecifications:passporttype, whitebackground35mmx45mm,datingfromthelast6months PASSPORT travel document must be valid at least 90 days after the expiration of visa and not olderthan10years;traveldocumentmusthaveatleasttwoconsecutiveblankpages; damagedtraveldocumentscannotbeaccepted. APPLICANT'S COVERINGLETTER mentioningthepurposeanddurationoftravel,listofattacheddocumentsandother usefulinformation. RESIDENCEPROOF Anofficialdocumentprovingstayinthecurrentjurisdiction.

TRAVEL MEDICALINSURANCE originalandonecopyofthecertificateofTMIissuedbyacompanyfromthelistofapprovedIndian insurance companies of the consular posts of Schengen states (handwritten certificates will not be accepted; the name of the insured should be written in Latin alphabet, TMI must be valid in the Schengenareaandfortheentiredurationofstay;theTMI'sminimumcoverageis30.000euroandit mustcovercostsofanyemergencymedicaltreatmentandrepatriationformedicalreasonsaswellas incaseofdeathrepatriationofthedeceased). ACCOMODATION originaldocumentconfirmingtheavailabilityinPolandofappropriateaccommodationfortheintendedperiodofstay (documentsshallcontaininformationaboutthecostofliving;thecostsforafullperiodormonthlypaymentmustbe indicated):certificateofregistration(zameldowanie),leaseagreement,decisionontheprovisionofthedormitory,other type of accommodationproof FLIGHT ITINERARY (RESERVATIONONLY)

  5. 8.MINORS Birthcertificate(ifapplicable,proofoflegalguardianship)apostilledbyMFA. CopiesofsignedIDdocumentsoftheparents/guardiansoftheapplicant. Notarizedcertificateofpermissiontotravelfromtheparent(s)/guardian(s)notaccompanyingtheminor duringthetravel,apostilledbyMFA,andwhennotinIndia,attestedbytherelevantauthoritiesinthecountryofresidence. Deathcertificateifoneorbothoftheparent(s)is/aredeadapostilled/attestedbyMFA. Courtverdictincasetheparentsaredivorcedand/oroneoftheparentslostcustodyofthechild,apostilled/attestedbyMFA. 9. DOCUMENTS CONFIRMING THE SUFFICIENT FINANCIAL RESOURCES TO COVER COSTSOFLIVINGANDRETURNTRAVELTOTHECOUNTRYOFORIGINORRESIDENCE applicantapplyingforanationalvisaforstudymustprovideadocumentconfirmingthathehassufficientfinancial resources tocover: Thecostofreturningtothecountryoforiginintheamountof2500PLN(minimum)tocoverreturningtothecountry oforigin(otherthanacountryneighboringPolandoranEU/EFTAmembers), Livingexpensescoveringtheplannedstay:therequiredminimumforeachmonthoftheplannedstayis701PLN/person, or528PLN/eachpersoninthefamily(ifapplicantwillstayinPolandwithfamilymembers). Cost of accommodation (documents must be submitted in a way the consul will be able to estimate the cost of living e.g. lease agreement, a decision on the provision of a dormitory, etc.). E.g. Applicant from India for a 365-day visa must have at least 10912 PLN (701 PLN x 12 months + 2500 PLN) to cover the cost of return and living expenses and additional funds to coverthecostofaccommodation.Ifthemonthlypaymentforadormitoryorrentinganapartmentis1000PLN,thenthecost ofaccommodationwillbe:1000PLNx12months=12000PLNperyear.Intotal,theapplicantneedsatleast22912PLN.

  6. 9.1. DOCUMENTSCONFIRMINGTHESUFFICIENTFINANCIALRESOURCES (OFAPPLICANTORSPONSOR)cannotbeissuedmorethanonemonthbefore submission of visaapplication Traveler’scheque. Certificateoftheamountofthecreditcardlimitissuedbythebank(anybanklocatedinIndiaorPolandoranother country isallowed) AcertificateofavailabilityofmoneyinthePolishorEuropeanUnionbankoracooperativesavingsandloanassociation locatedintheterritoryoftheRepublicofPolandorEuropeanUnion(itcannotbeabanklocatedinIndia) Adocumentconfirminggrantingofanationalorforeignscholarship. Acertificateofemploymentwithmonthlysalarystatement

MINORSANDADULTSFINANCIALLYDEPENDENTONSPONSORS: Letterofsponsorship-itshouldbenotedthatthesponsorsarerequiredto coveralltravel,livingandaccommodationexpensesofthepersonapplyingforthevisa: Fromtheapplicant’sparents-thesponsor’slettermustbecertifiedbyanotary. Fromathirdperson-admissibilitywillbeassessedbytheconsul;thesponsor’slettermustbe certifiedbyanotaryandapostilledbyIndianMFA Documentsconfirmingtheavailabilityoffinancialresourcesofthesponsor(indicatedatp.9.1) Documentconfirmingrelationshipwiththesponsor(e.g.thefamilyties)-theexactsamespellingofsponsor'sname inthedocumentconfirmingtherelationshipwiththesponsor,inthesponsor’sletterandinthedocumentsconfirming thefinancialresources,isrequired,e.g.ifparents'namesarenotcorrectlyorfullyindicatedintheapplicant’spassport, another official document (in Polish or English) confirming the family ties (e.g. birth certificate) apostilled by the MFA shouldbesubmitted.Pleasenotethataffidavitswillnotbeconsideredofficialdocumentsinthisregard.

DOCUMENTSCONFIRMINGTHEPURPOSEOFTHETRIP Theoriginalcertificateoftheunitconductingthestudiesonadmissiontostudiesoronthecontinuationofstudies inaccordancewiththeformatsetoutintheRegulationoftheMinisterofScienceandHigherEducationfrom 24th of September2018. Proofofpaymentforstudies-issuedbytheeducationalinstitution;ifstudiesarefreeofcharge,aconfirmationfrom the university isrequired. Documentsattestingpreviouseducation(originalsshouldbepresentedatthecounter): Secondary school leaving certificate - if the applicant applies for a visa on the basis of a university certificate on admissiontothefirstyearofstudies/preparatorycourse,onemustsubmitthesecondaryschoolleavingcertificate apostilled/attested bythe MFA. Certificates/diplomasofgraduation-iftheapplicantappliesforavisaonthebasisofauniversitycertificateon admission to the second and third cycle studies, he/she must provide the certificate apostilled by the MFA / diploma ofgraduation.

  9. 11. ADDITIONALDOCUMENTS applicantsareallowedtosubmitadditionaldocumentsthattheyconsiderusefultoexplain/justifythetrip.E.g. English Language test such as: IELTS {International English Language Testing System} (Band 6.0), TOEFL {Test of English asaForeignLanguage}(minimumscoreof550pointsforthepaperBasedTestor213pointsfortheComputerBasedTest), andCAE(CambridgeEnglish:Advanced)arehighlyrecommendedasaproofofpossessingthenecessarylanguageskills forundertakingstudiesconductedinEnglish. 12. PASSPORT DATA PAGESCOPY Onecopyoftheapplicant'spassportdatapages(thefirstandthelastone). CopyofthepageswithPolish/Schengenvisas,ifissued(ifthevisaswereinthepreviouspassport copyofthatpassport’sdatapages-thefirstandthelastone).
