This case involves a 75-year-old female patient diagnosed with extra-mammary Paget's disease of the vulva, resulting in significant tissue loss. A bilateral pedicled flap (PBIF) reconstruction was performed using skin paddles designed along the bilateral gluteal and inguinal folds to cover the surgical defect. The size of the left PBIF was 18 x 4 cm, and the right was 18 x 5 cm. The elevation of the right flap was marked with a perforator indication, and primary closure of the donor sites was achieved along the gluteal and inguinal folds postoperatively.
