The Back to School Packet • This packet contains necessary information and forms that must be reviewed by both parents and students. • Students must follow specific instructions for ensuring the required forms are completed and returned.
Forms that must be returned… • Students/parents will keep all of the materials at home except for the following pages: • LAMS Code of Behavior Agreement • LAMS Student/Parent/School Compact • Parent and Student Annual Legal Notice (Pages 2, 22, 23, and 24) • Picture/Audio/Webpage Release Form • Student Residency Questionnaire • LAMS Emergency Contact Form • PTSA Membership Envelope (optional)

When all forms are completed and signed…. • Put all completed forms back into the orange envelope. Keep all other materials at home for parent/student reference. • Return completed forms to the student’s homeroom teacher no later than the assigned due date.
