    3. 3 Vocabulary Pain Scales Numeric Rating - rated by patient on a scale of 0-10 Unable to Use Pain Scale (Behaviors) patient is unable to verbally express pain - nurse is able to document behaviors that suspect pain after other reasons are ruled out Non-verbal Advanced Dementia (PAINAD Scale) patient is unable to rate pain because of dementia-nurse documents in five categories to determine a PAINAD score Gen View - screen which provides the last charted documented assessment
