Comprehensive Coverage is Critical • Health care is more costly in the US • Health insurance is a necessity • Unexpected accidents and illnesses do occur • Uninsured & under-insured are at high risk • Many sacrifice treatment or face financial hardship • Students without health insurance may have to drop out of school

  8. Mandatory Health Insurance • UW-Madison has mandatory health insurance requirements • All international students & visa dependents must enroll in SHIP or file a qualifying Waiver Application • SHIP meets or exceeds Federal requirements and Wisconsin State insurance mandates

What is SHIP? • SHIP is a comprehensive student health insurance plan • SHIP office located in University Health Services (UHS) • Coverage includes: • $1,000,000 maximum lifetime benefit • Vaccinations and immunizations • Prescription coverage • Medical evacuation • Access to local and nationwide hospitals and clinics • Annual eye exam

What do you need to do? • International students registered for class must: • Purchase SHIP for yourself and any visa dependents • By February 14 deadline • If you purchase SHIP after the deadline, you will be • charged a $100 late fee! OR • Submit a qualifying Waiver Application • By February 14 deadline • If you file a waiver after the deadline, you will be • charged a $100 late fee!

  11. SHIP Enrollment • Enroll in SHIP on-line at: www.uhs.wisc.edu/ship • Select SHIP and • Enroll in person at: 333 East Campus Mall – 7th Floor • At our office 9am-5pm, Monday through Friday • Pay by VISA/MasterCard, check or exact cash

  12. What happens after enrollment? • You will receive a SHIP membership card by mail • Keep your membership card with you at all times • Used for medical appointments and pharmacy visits • You can check your SHIP status on-line anytime

  13. Who do you call for an appointment? • University Health Services provides primary carefor all SHIP members except dependent children • UHS is the only provider with 100% SHIP coverage • To make an appointment at UHS call: (608) 265-5600

  14. Where is UHS?

Coverage beyond UHS • UHS is not open evenings and weekends and does not provide emergency care or hospitalization • If it is an emergency and you require immediate medical attention call 911 • Local providers for SHIP members include Meriter Hospital, UW Hospital and Clinics and St. Mary's Hospital. • All SHIP provider information can be accessed from our web-site: www.uhs.wisc.edu/ship

  16. Billing and Explanation of Benefits • You will receive a bill 3 – 4 weeks from the date of your service • You will also receive an explanation of benefits from the claims office • If you have ANY questions, contact the SHIP office

Waivers • Must meet qualifying criteria on Waiver Application • All other applications will be denied • Individual plans are not accepted • Current Waiver Application is available on-line • February 14 deadline • If you file a waiver after the deadline, you will be charged a late fee of $100 and the cost of SHIP for any uninsured period!

  18. Automatic Waivers • Must meet the following criteria to be automatically waived from SHIP: • Be the primary member of a Wisconsin State Group Health insurance plan (Dean Health Care, Physicians Plus, Unity, GHC, Standard Plan or WEA Trust PPP) • Coverage must take effect on or before February 1st of this year

  19. Remember… • By February 14, you must: • Purchase SHIP for yourself and any visa dependents OR • Submit a qualifying Waiver Application

  20. If you miss the deadline… • You will be charged a late fee of $100 in addition to any required premiums • You will be automatically enrolled in SHIP and invoiced for the outstanding balance • An academic hold will be placed on your student record until you have submitted a payment.
