Leptospirosis • It is a generalized infection caused by spirochetes (genus Leptospira) • Zoonotic in nature and enters through the moist and abraded skin or through mucous membranes • Primary lesions damages the endothelial lining of small blood vessels, resulting to ischemic damage to the liver, kidneys, meninges and muscles • Presents with an initial septicemic phase (2-7 days) after a period of incubation (7-12 days)(, which may be followed by a brief period of well-being and a second symptomatic or immune phase

  3. Clinical Manifestations • Symptoms • Non specific signs and symptoms (70 %) • Meningitis (20 %) • Hepatorenal dysfunction (10 %) Characteristic • Onset is sudden • Biphasic course

  4. Reasons for Considering Leptospirosis • The previous of contact to water that can be probably infected with the organism (10 days PTA) • Clinical progression of the symptom fits in the timeline of the disease progression

  5. Reasons for Ruling Out Leptospirosis • Absence of other important clinical findings for the disease such as myalgia, headache, vomiting and nausea (in septicemic phase); and meningitis and uveitis (in immune phase) • Characteristic of the fever is different (the patient has a continuous fever since onset of symptoms vs. biphasic pattern in Leptospirosis) • Location of the cutaneous lesion (patient has facial lesions vs. truncal lesions in Leptospirosis)
