Level 2 Services Referrals from primary care Referrals from secondary care Referrals to the specialist nurse clinics can be made through the Choose & Book • Type 2 diabetes • patients with sub optimal glycaemic control, not responding to treatment at level 1 • patients who require insulin initiation (see separate guidance for eligibility) • patients whose clinical indicators appear to be outside normal parameters • Type 1 diabetes • patients who are not currently receiving care at level 3 whose care can no longer be achieved optimally at level 1 • Desmond Structured Education • Please refer all newly diagnosed patients with Type 2 diabetes for DESMOND (Diabetes Education for Self Management Ongoing and Newly Diagnosed) structured education. • In Somerset, courses are run at regular intervals in different venues around the county. The course lasts for one day and can be booked through Choose & Book. Patients can choose their preferred time and location to attend the course. • Type 1 and 2 diabetes • stabilised complex cases whose care cannot yet be met at level 1 • patients discharged from in-patient care who require follow-up but not necessary at level 3 • patients on newer therapies for ongoing monitoring • pre and post pregnancy patients with gestational diabetes or type 2 diabetes whose care can be managed at level 2 Community Diabetes Specialist Nursing and Dietetics Service Confirmation of diagnosis for those outside normal parameters Treatment and management planning for patients with sub optimal glycaemic control at Level 1 Patients with a BMI <18 or with complex weight management needs Insulin initiation Liaison with community matrons over patients with complex needs NB Choose & Book is the preferred referral method for practices.
