You have performed a patient care hand-wash. You have performed a patient care hand-wash. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق • Hypovolemic shock is a condition caused by a sudden decrease in volume of fluid in the body's blood circulatory system • په بدن کی د وینی د کموالی څخه رامینځ ته شویشاک دا هغه حالت دی چی په ناڅاپی توګه د وینی په دورانی سیستم کی د وینی مقدارکم او را لویږی چی معمولآ د محاربی په ډګرکی د وینی ضایع کول او سوزونی دا ډول پیښی را منیځ ته کوی. • Signs and symptoms of hypovolemic shock include the following: • د وینی د لږوالی له امله د رامینځ ته شوی شاک یا کوما اغراض او علایم په لاندی ډول دی: • Rapid or severe bleeding • چټک یا شدیده خونریزی. • External bleeding from a visible wound. • خارجی خونریزی چی زخم یی د لیدلووړ دی. • Internal bleeding from a trauma with no visible wound (usually seen as swelling or discoloration) • داخلی خونریزی هغه خونریزی چی زخم یی په سترګو نه ښکاری(معمولآ پړسیدلی وی او یا یی رنګ تغیرکړی وی) • Severe burns (second and third degree burns over 20 percent or more of the casualty's skin surface). • شدیدی سوزیدنی( سوختګی) ( دوهمه درجه یا دریمه درجه سوختګی چی د سوزیدنی درجه د ۲۰ فیصد یا ترهغی هم پورته وی شدید سوزیدنه بلل کیږی )

INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق • Signs and symptoms of hypovolemic shock include the following: • د وینی د لږوالی له امله د رامینځ ته شوی شاک یا کوما اغراض او علایم په لاندی ډول دی: • Anxiety. (This is an early sign of shock. As fear increases, the heart rate • increases, which usually causes the casualty's overall condition to deteriorate.) • اندیښنی او تشویشونه . ( دا دشاګ لومړنۍ نښه بلل کیږی. کله چی ویره زیاتیږی، دزړه ضربان سرعت مومی، چی په عمومی ډول د حالاتو دخرابیدوسبب ګرځی) • Confusion. (The casualty may not understand his surroundings and take inappropriate actions. This condition is generally caused by lack of oxygen to the brain. • ورخطایی . (The casualty may have a strong desire to move about or leave.) • ناراحتی : (ناروغ ښایی د حرکت کولو ډیره ارزو ولری ). • Agitation. (The casualty may become agitated to the point of violent behavior and attack people around him.) • تحریک (لمسوونه) ناروغ ممکن تر دی برید تحریک شوی وی د هغه شاوخوا اشخاصو باندی تیری یا حمله وکړی. • Changes in the level of consciousness. (The casualty may quickly go from fully alert to unconscious.) • د هوښیارتیا په کچه کی بدلون . ناروغ ممکن په ډیره بیړه سره د پوره هوښیارتیا څخه د بی هوښۍ خوا ته لاړشی.

(if appropriate conscious versus unconscious) الف . د ناروغ د نوم پوښتنه وکړۍ. (وګورۍ چی په هوښ دی او که بی هوښه دی) b. Check the identification band against the patient's chart, as appropriate. ب. د اړتیا په صورت کی دناروغ معلومات د دوسیه سره تطبیق کړۍ. c. Explain the reason for IV therapy. ج. د وریدی زرقیاتو د درملنی سببونه واضح کړۍ. d. Ask about any known allergies to such things as betadine or medication. هـ. د هر ډول درملو لکه بیټاډین یا نورو درملو سره د حساسیت ( الرجی ) په هکله چی دوړاندی معلوم شوی وی پوښتنه وکړۍ. 2. Inspect the equipment looking for defects, expiration date, and signs of contamination. ۲. تجهیزات د عوارضو ، د انقضآ تاریخ ، او یا هم د ککړتیا له پلوه معاینه کړۍ . a. IV fluid of choice (check medical officer's order). Discard containers that have cracks, scratches, leaks, , condensation, or fluid which is not crystal clear and colorless. الف . ټاکل شوی زرقی مایعات ( دمسؤل ډاکټر توصیه وګورۍ) او هغه قطۍ چی زیانمنی یا ککړی شوی هغه وړاندی وغورزوۍ.

  14. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق b. IV injection set. ب. د زرقیاتو ( سیروم ) سیټ . (1) Spike, drip chamber, tubing, and needle adapter. Discard them if there cracks or holes or if any discoloration is present. ۱) د څڅیدنی(څاڅکو) چمبر، ټیوبونه او د ستنی اډاپټر. که چیری دا وسایل خراب وی او یا زیانمن شوی وی ایسته یی وغورزوۍ. (2) Flow regulator. Inspect the flow regulator and ensure that it tightens. ۲.د جریان ریګولاتور( تنظیم کوونکی ) د جریان ریګولاتور معاینه کړۍ او وګورۍ چی کلک تړل شوی وی. (3) Catheter-over-needle. Discard them if they are flawed with barbs or nicks. ۳. ستن لرونکی کتیتر. که چیری د کتیتر څوکه یا بل کوم ځای زیانمن شوی وی ا یسته یی وغورزوۍ NOTE: Place the stand to the side of the patient and close ، to the IV site. نوت- د وریدی زرقیاتو د اخیستلو لپاره سټینډ دناروغ سره په نږدی ځای کی نصب کړۍ.

  17. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق 3. Prepare the equipment. د تجهیزاتو چمتوکول . a. Move the flow regulator 6 to 8 inches below the drip chamber and tighten/close it. الف. د جریان تنظیم کوونکی د څاڅکو د چمبرڅخه د ۶ ــ ۸ انچه لاندی یو سۍ او هلته یی کلک وتړۍ . b. Remove the protective covers from the spike and from the outlet of the IV container. ب. دسیروم دخروجی مجرا څخه محافظتی خولۍ ( خوندی خولۍ ) را لیری کړۍ. c. Push the spike firmly into the container's outlet tube. ج. د قطۍ په خروجی ټیوب باندی میخک کلک وتړۍ. d. Hang the container at least 2 feet above the level of the patient's heart, if possible. د. د امکان په صورت کی د سیروم خلته د ناروغ د زړه څخه دوه فوټه په اندازه پورته ځوړند کړۍ.

  18. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق 3. Prepare the equipment continued. ۳. د تجهیزاتو چمتوکول د مخکی سلایډ په ادامه. CAUTION: Do not touch the spike or the outlet of the IV container. پاملرنه : د وریدی زرقیاتو د قطۍ د خارجی مجرا سره تماس مه کوۍ. e. Squeeze the drip chamber until it is half full of the IV fluid. هـ . د څاڅکوچمبرته ترهغی فشارورکړۍ چی وریدی مایعات نیمایی ته ورسیږی. f. Prime the tubing. ز. د ټیوب ډکول .(د سیروم سیټ) NOTE: An IV bag container may be placed under the patient's body if there is no way to hang it. NOTE: Ensure that all air is expelled from the tubing. نوټ : د پایپونو څخه د هوا د وتلو په هکله ډاډ لاسته راوړۍ. (1) Hold the tubing above the level of the bottom of the container. ۱. د قطۍ لاندینۍ سطحه د ټیوب څخه پورته وتړۍ. (2) Loosen the protective cover from the needle adapter to allow the air to escape. ۳. دهوا د وتلولپاره د ستنی د اډاپټرڅخه خوندی سرپوښ لیری کړۍ. Choose the most distal and accessible vein of an uninjured arm or hand. ب. د سالم یا غیرټپی شوی لاس ورید انتخاب کړۍ. c. Avoid sites over joints. ج.د مفصلونودځای څخه ډډه وکړۍ. d. Avoid veins in infected, injured, or irritated areas. د. د ورید دتخریش شوی، ټپی شوی ، یا مکروبی شوی ځای څخه ډډه وکړۍ. e. Use the non-dominant hand or arm, whenever possible. هـ . د امکان په صورت کی د غیرمسلط لاس څخه کارواخلۍ.

  22. INITIATE AN INTRAVENOUS INFUSIONد ورید له لاری د زرقیاتو تطبیق 4. Select the infusion site. ۴. د زرقیاتو ځای وټاکۍ. f. Select a vein large enough to accommodate the size of needle/catheter to be used. و. داسی یو ورید انتخاب کړۍ چی د ستنی او کتیتر د سایزسره کافی برابروالی ولری. CAUTION: Do not use an arm that may require an operative procedure. پاملرنه: هغه لاس کی چی دعملیاتی پروسیجرونو سره سمون ونه خوری استعمال نه کړۍ. .

  23. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق 5. Prepare the infusion site. ۵. د زرقیاتو دځای چمتوکول. a. Apply the constricting band. الف . د تړلو تسمه په کار یوسۍ. (1) Place the tubing around the limb, about 2 inches above the site of venipuncture. Hold one end so that it is longer than the other, and form a loop with the longer end. ټیوب د غړی شاوخوا د پیچکارۍ د ځای څخه د ۲ انچه په اندازه پورته ځای په ځای کړۍ. یو سر یی ونسیۍ او بل سر یی اوږد اوغوټه ورواچوۍ. NOTE: When applying the constricting band, use soft-walled latex tubing about 18 inches in length.   24. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق 5. Prepare the infusion site continued. ۵. د زرقیاتو دځای چمتوکول. دمخکی سلایډ په ادامه. NOTE: When placing the constricting band, ensure that the tails of the tubing are turned away from the proposed site of venipuncture. نوټ : کله چی د تړلوتسمه ځای پرځای کوۍ . وګورۍ چی د ټیوب ورستۍ برخه د د ستنی د دننه کیدو دځای څخه لیری وی. (3) Apply the constricting band tight enough to stop venous flow but not so tightly that the radial pulse cannot be felt. ۳) د وریدی جریان د توقف لپاره تسمه وتړۍ لیکن دومره ټینګه یی هم مه تړۍ چی د نبض ټکان احساس نه شی. (4) Tell the patient to open and close his fist several times to increase circulation. ۴) د وینی د جریان دزیاتوالی لپاره ناروغ ته ووایی چی څوځله موټی بند اوخلاص کړی.

  25. INITIATE AN INTRAVENOUS INFUSION د ورید له لاری د زرقیاتو تطبیق 5. Prepare the infusion site continued. ۵. د زرقیاتو دځای چمتوکول. دمخکی سلایډ په ادامه. b. Select a prominent vein. ب. یو برجسته ورید انتخاب کړۍ. c. Tell the patient to close his fist and keep it closed until instructed to openthe fist. ج. د ناروغ څخه وغواړۍ چی لاس مو موټی او ترهغی چی درته نه وی ویل شوی لاس موموټی وساتۍ. d. Clean the skin over the selected area with 70% alcohol or betadine, using a firmcircular motion from the center outward. د. د پیچکارۍ ځای په بیتادینو یا الکولو( ۷۰٪) الکولو باندی ومینځۍ. e.
