Filing Replacement Claims • Material needed: membership agreement, copy of the Lease Agreement, payment history, print screen of invoice cost of lost merchandise, & SIGNED claim form. • Burglary claims - Customer must bring photos that show forced entry and a Police Report that supports. Material must be submitted within 10 days of loss • Fire Claims – Customer must bring photos that show fire damage and a report from the Fire Department. Both must be submitted within 10 days of loss

The Stores that exceed their goals by the greatest percent will be considered the winners. • Prize Description: The Stores in each category below who exceed their goals by the greatest percent will win $750 (a $400 gift card to the manager and $350 to be divided between the assistant managers, sales staff, and account managers. • Incentive Plan Description • The incentive will run for four months beginning July 30, 2011. The stores will be divided into five categories.
