Why New Ways of Working? • To review the form and function of the ABHB Locality structures following the establishment of the Health Board in Oct 2009 • To maintain locality focus – working in partnership with Local Authorities, GPs, Third Sector and other stakeholders in developing Single Integrated Plans.

Functions of Primary Care and Networks Division • Lead the Partnership Agenda at Locality level, including Single Integrated Plan • Manage all Section 28as, Service Level Agreements with LAs and Third Sector • Develop Neighbourhood Care Networks to drive the service transformation agenda across the Health Board • Manage all Independent Contractor issues: contracts, performance workforce, professional issues • Support effective prescribing in Primary Care • Primary Care Estate developments • Manage Prisons in Usk and Prescoed • Manage the Mental Capacity Act (Deprivation of Liberty Safeguarding Function for Health Board and 5 LAs.

Neighbourhood Care Networks in Caerphilly • 3 - North, South and East • Clinically led to reflect and integrate role of primary care in the delivery of all services in the community • Membership being reviewed: include GPs, CRTs, Community Health and Social Services, Acute Consultants, Third Sector, Housing, Communities First

Neighbourhood Care Networks Principle function: To Optimise and Improve Care to Local Communities: • Through Collaborative working • Finding local solutions to services delivery issues • Maximising use of all resources • Whole system approach to patient centred pathways (across all health, community and social care settings • Delivery of single integrated plan

Partnership Principles • Community strategy for Caerphilly still remains a key foundation for partnership activity • Four Community Plan themes (Education for Life, HSCWB, Regeneration and Living Environment) remain unaffected by restructuring • Effective working relationships continue to be developed through the Primary Care & Networks Division and linked through the Neighbourhood Care Networks • Senior Partnership Staff to be based with Local Authority Corporate Teams to strengthen cross organisational working • All planning for health related services to reflect the ABHB 5 year Plan, Divisional Operational Plans and NCN priorities. • Key links for Partnership Planning (Single Plan and wider community engagement, third sector contracting etc) through Primary Care & Networks Division and Locality Heads of Partnerships & Networks.
