  1. Local Isolation Report and Trigger Tool Pete McDonnell Strategic Commissioning and Market Development

  2. Isolation Report • Produced for the Older Persons Partnership Board • Initial report looked at key statistics and what services are available in the borough • Isolation sub group established • We also then surveyed frontline social, healthcare and community professionals and local older people to get a better picture of isolation • Idea for a conference in December • Trigger and referral tool for professionals and the community organisations etc

  3. Key findings • 6 out of 10 older people are classed as living in “Lone Pensioner households” (approximately 12,000 people). • Over 50 per cent of over 65s live in first floor or above properties. • Brompton, Hans Town and Royal Hospital have the highest numbers of lone pensioners (Royal Hospital figures are disproportionate due to the 200 Chelsea Pensioners). • There are also 11 wards with 500 to 700 lone pensioners across the borough. • Over 70% of professionals dealing with older people said they visit lone pensioner households • More White UK • More Females • Those aged 75 plus more likely to experience isolation

  4. The main reasons for isolation • No local family • Poor or deteriorating mobility • Depression • Loss of confidence • Bereavement (partner, family, friend, neighbour) • Poor and inaccessible housing

  5. Digging deeper Housing issues: • Poor quality flat with accessibility issues • Guaranteed rent, so landlord not prepared to renovate • Unsuitable bathroom • Young people move into block, community spirit lost

  6. Molly aged 88 • In poor 3rd accommodation • Has a fall • Loses confidence • Stops going to church • Can’t visit her only friend via the bus • Starts to isolate herself and becomes depressed

  7. Services • 13 churches and faith groups support older people. • The Council’s Adult Social Care funds nine services that support, or have an element that supports, isolated older people, supporting around 800 to 900 people weekly. • There are four day centres for older people in the borough. • Corporately the Council funds at least six providers that support older people. • Most support is in the voluntary sector. • Social landlords also provide floating support to older people. • Community Mental Health Teams support isolated and vulnerable older people with mental health issues.

  8. Comments and ideas please! By Friday 14th Jan 2011 to Peter.McDonnell@rbkc.gov.uk

  9. Trigger Tool • Whose it for? Frontline professionals, community orgs etc. The Idea • Online tool • Quick assessment • Instant referral for further advice or to refer the person (with consent)

  10. What does it look like?

  11. When is it available and where is it? • The full working version is available from early January 2011 • A link will be sent to all attendee’s of the conference for the page of the Council’s People First web pages
