Healthcare, Community Services & Provider Directory • But this is more than Health you say!!! • Well, contrary to many opinions Health cannot & does not operate in isolation • People must be able to be referred into and outside of the Health System as well as within the Health System • The primary function of the Directory is to support Referral

  4. Healthcare, Community Services & Provider Directory • Scenario (Australia) • Tom is single and a casual labourer who originally hails from Perth, however now lives & works in Melbourne. His family is still in Perth. He rents a flat (currently behind in rent) and has no insurance of any sort. On Saturday he goes recreational fishing • While fishing Tom accidentally stands on a small spike which goes through his shoe & into his foot. He pulls it out & carries on • On Monday he goes to work, obviously in severe discomfort

  5. Services & Provider Directory Scenario (cont) • On Tuesday Tom goes to work in more discomfort. The foreman sends him home sick • He makes an appointment with a local GP, earliest time he can get is Wednesday afternoon • Tom presents to the GP who observes infection, gives a certificate for the week and a prescription for antibiotics, which Tom doesn’t get filled due to the cost

  6. Services & Provider Directory Scenario (cont) • By the following Sunday Tom is in constant pain with his foot. In desperation he catches a taxi to the nearest Hospital Emergency Department • Late Sunday evening it is determined Tom will need some surgery and strict infection control. Tom is admitted into the Hospital • Surgery is booked for Wednesday

Services & Provider Directory Scenario (cont) • 1 week following surgery Tom is served with an eviction notice from his landlord, giving him 24hrs to pay the outstanding rent – money which Tom does not have • 2 weeks after surgery Tom is discharged with walking aides and requiring at least 1 more week of convalescence. During the discharge process Tom makes the Admin Officer aware of his dire circumstances

Services & Provider Directory Scenario (cont) • The Admin Officer responsible for Tom accesses the Services Directory to find the GP details to send his Discharge Summary electronically • The same Admin Officer then accesses the Service Directory to find an Organisation (NGO) close buy that includes the Supported Accommodation Assistance Program (SAAP). The Admin Officer generates and e-Referral to this Organisation

Services & Provider Directory Scenario (cont) • The Admin Officer then uses the Services Directory to help generate a e-Referral for home nursing and an e-Referral to Local Government for meals on wheels • Tom then presents at the SAAP NGO. They give him a rooming house voucher for 5 nights and 4 taxi vouchers, and generates an e-Referral to the rooming house using the Services Directory

Services & Provider Directory Scenario (cont) • The SAAP Organisation then refers Tom (electronically) to a Public Housing Office, for longer term temporary accommodation gives him the details and sends him to the rooming house • They then send the completed e-Referral back to the Hospital Discharge Department which includes his new temporary address • The Discharge Department then sends e-Referral updates (new address) to the Home Nursing Service and Local Government
