  3. Go to www.surveymonkey.com

  4. Click Sign Up Free

  5. Create an account by filling up the form

  6. Click Create Account

  7. Enter your Survey Title

  8. Add a theme for your survey form

  9. Create a question by clicking add question

  10. Type your question

  11. Type your answer choices

  12. Select format for question and answer choices

  13. After designing the survey form click save

  14. You may put additional questions just click add question

  15. Click Preview Survey to check your survey form

  16. Review your Survey Form

  17. Choose how to collect the responses from the options given

  18. Then Click Next Step

  19. You may change the setting and restrictions of the survey

  20. Click Change Setting or Change Restriction for adjustments

  21. Click Analyze Results

  22. To view the feedbacks of the respondents
