Nutrition Data Collection and Community Support Evaluation

This document outlines a survey conducted to assess cereal consumption habits and demographic information among adults. It includes questions about frequency of whole grain cereal consumption, household size, race and ethnicity, participation in nutrition assistance programs, and household income levels. The aim is to evaluate community nutrition and identify needs for support services. Responses will help guide nutrition initiatives and improve health services in Arizona.
