  2. South Laboratory • Opened January 2012 • Clinical Biochemistry, Haematology and Blood Transfusion • Opposite rear entrance to AMAU • Connected by pneumatic tube to new hospital • Electronic requesting being rolled out

  3. Pneumatic tube system • Currently only South Laboratory • Clin Biochem/Haem emergency 911 • Routine 913/914 • Blood Transfusion 912

  4. Pneumatic tube system • No Microbiology (summer 2012) • No blood cultures (summer 2012) • Allow yellow-topped gel tubes to clot 5 minutes before sending • Do not send routine samples to 911 • Ensure that swivel opening clicks shut

  5. DON’T . . . DO . . . • Make urgent requests unless samples • are truly urgent / emergencies • Complete form fully and legibly • Complete your own details and • the name of the Consultant • Make “add-on” requests a routine • Use addressograph labels for sample tubes • Give accurate clinical information • Expect that samples sent out of hours • will be processed that day (unless urgent) • Notify the lab by phone when sample • results are required urgently • Seek advice if unsure about process • or unsure about the interpretation. • Avoid unnecessary telephone • calls for results Specimen acceptance policy in place, therefore all details for patient correct ID are required before samples are accepted and processed. Blood transfusion • Request form must be signed by medical practitioner as considered a prescription. • Person taking sample, e.g., nursing staff must also sign form. • Sample tubes must be fully labelled by hand at bedside confirming details with patient or wristband. • May use addressograph label on form. • All details must be given on request form. • The “Urgent” (within 40 minutes) and “Emergency” (within 15 minutes) tick boxes are for use ONLY if the patient has a life threatening need for blood transfusion. This must also be indicated by a phone call to the blood bank staff.
