SunTel 350 Requirements • Application Requirements • - SunTel Application Form • - 1 month Advance Monthly Service Fee (AMSF) • - Proof of Identification & Proof of Billing  Proactive post-call-outs to validate billing address will be conducted • - For existing subscribers, account check-up will be conducted. His/her account must be current and without delinquency history during the last 6 months(No redirection or TD)

PROOF OF BILLING (Any of the following may be submitted) • Utility Bill Statement (Electricity, Telephone, Water or Cable/ Internet) • Credit Card Statement (SOA must be paid and most recent month) • Insurance Premium Statement of Account (current) • Notarized Lease Contract (if rented) NOTE : Submitted Statement should be current (should not be more than 45 days old) and with NO overdue balance. PROOF OF INCOME (Any of the following may be submitted) Latest Income Tax Return(ITR – BIR Form No 2316) with Basic Gross Annual Income of P153,000 duly stamped by BIR and latest 2 half month original pay slips Certificate of Employment newly issued with compensation, tenure, department and position issued by HRD with attached current pay slip with at least Php12,750/month). Standard company letterhead is required and latest 2 half month original pay slips. Income component basic, PERA, COLA, allowances. Validity 60 days only upon issuance Credit Card Statement (last 2 months paid and with CL of at least P28,000). Allowed PRIMARY CARDHOLDER ONLY. Attached Proof of payment if already due. Presentation of latest SOA together with the credit card is required. At least 25% available CL balance or MSF plus handset cost, whichever is higher. (Except BPI, Citibank, EQB and Amexco). Tenure, CL, usage, and payment history combined will serve as basis for assessment of financial capacity NOTE : Statement must be most current and with no overdue balance.
