Returning students will be charged a $15 registration fee. Children will be accepted on a first come first serve basis and this packet must be received with the registration fee in order for the enrollment process to be complete. If the classes are full you will be put on a waiting list and be notified of the earliest vacancy. All tuition payments must be paid by the 1st of the month. You will receive monthly tuition invoices near the end of each month notifying you how much is due. If a payment is made after the 1st of the month, a late fee of $5 will be added to the following month’s tuition. If tuition is not paid by the 5th of the month, your child will not be allowed to come to class. Not paying the late fee the following month may result in your child being dropped from Lil Da Vinci Academy. You have the option to pay online at lildavinciacademy.weebly.comor in person with cash or check. If paid in person, the tuition must be paid on the preschool day before the due date to avoid incurring a late fee. Online payments will only be available until the due date. Payments made after this date must pay in person. MWF groups will be every Monday, Wednesday Friday and TTH groups will be every

If classes are canceled at any time, you will be notified and the class will be made up with extended class time on another day. If so, please list: Please list anyone who may be picking up or dropping off your child: Illness: We strive to provide your children with a safe and healthy environment at Lil’ Da Vinci Academy. If your child is sick enough to affect the other children, you must keep them home. If your child becomes sick while at preschool, we will notify you and you may come pick them up. Please do not bring your child to preschool if they have any of the following symptoms: Vomiting Fever Diarrhea Sore Throat Severe Coughing Questionable Rash or Bumps Please keep them home until they are symptom-free for 24 hours. We will not administer any medication to your child. Hours of Operation Lil’ Da Vinci Academy runs from 9:00 A.M. to 11:00 A.M. and 1:30 P.M. to 3:30 P.M. MWF or TTH. Your child may arrive up to 10 minutes before preschool starts and stay up to 10 minutes after preschool ends before incurring any additional fees. If there are any extenuating circumstances or

Fees will be added based on the length of time the child stays late. Withdrawal/ Dismissal from School Please notify Kari one month in advance if you are to withdraw your child from preschool so that she may invite another child from the waiting list to attend the preschool. Be courteous to those who may be waiting to join our preschool. If your child misses more than two consecutive weeks of school without notifying Kari, he or she may be asked to leave Lil’ Da Vinci Academy. Snack and Other Food Provisions Please have your child eat breakfast/lunch before coming to preschool. We will be participating in many hands-on activities in which your child needs as much energy as possible. Occasionally activities will include food or a snack to teach a concept. The children will be taught and participate in cleanliness and healthy habits such as hand washing. Discipline and Guidance As the teacher and care giver, I will model positive methods of guidance which will demonstrate self control and problem solving skills. Physical abuse or corporal punishment will never be used in preschool. Your child will instead be removed from situations or placed in time-out if they are unable to follow the rules set out at the beginning of preschool. If incidents occur, they will be brought up in confidence to the parent and discussed so that the undesired behavior may be stopped. If a child’s behavior can not be adjusted, you will be asked to remove your child from Lil’ Da Vinci Academy. Field Trips During the school year we will be participating in a number of field trips. The only two trips that have an additional fee are the trips to the Zoo Boise and the Discovery Center in Boise. The zoo trip will be during the school year the and the discovery trip will be during the summer. The costs are below and must be paid in cash the at the beginning of the month in which the trip is taken. Please let me know which of these trips you plan on having your child attend so I may make the arrangements in time. Zoo Boise Trip ($3.50 for admission and sack lunch) Summer Trip – to be determined ($5 or less) Parents who would like to assist in these field trips will also be charged the above fee. Any additional children they bring to these events are welcome! The fees will apply to those who are over the age of three. Do you plan on attending one of these events? If so which one(s)? Are you planning on bringing additional children to either of these field trips?

  4. T-shirt Order Form Each child will be given a preschool t-shirt at the time of first enrollment. If you would like additional shirts (for example: if you lose the first one, if your child grows out of his/her current shirt, or to get one for another child) You may order additional children’s shirts for $9.00. *The XS are pre-shrunk. Parents who plan on attending field trips and would like to have a Lil’ Da Vinci Academy shirt can also purchase one for $9.50. Any parent who would also like to support the preschool and purchase a shirt is more than welcome. The child shirt measurements are below. The adult sizing chart is below. Fill out this order form for your child’s shirt and any additional shirts you would like to order. Total Cost (If no additional shirts are ordered leave blank): If you choose to order additional shirts, you may make this payment separate from the registration fee. This fee is due no later than the end of September. If you wish to order the shirts before the field trips, let me know and we can make arrangements.

  5. Signatures to Agreement Photo Release: I give Kari Shuey/ Lil Da Vinci Academy permission to post my child’s picture on her website(s) as it relates to appropriate preschool activities. I understand that Kari will use her discretion and will not post any obscene or harmful images of my child. I understand that if there is a picture I would prefer not to be posted, I can notify Kari and it will be taken off the website(s) immediately. Signature of Parent or Guardian Date Contractual Agreement: For the services listed in this agreement, and in accordance with the terms and conditions of this agreement, I agree to pay Kari Shuey/ Lil’ Da Vinci Academy the monthly tuition amount of ($85.00) or ($65.00) by the 1st of each month. If I am late on my payment, I will be charged a $5.00 fee for the following month’s tuition. (Circle the tuition amount) I agree to pay a registration fee of ($35.00) or ($15.00) - circle which amount applies to you - and understand that it ensures my child a position in the: (check your preferences) Monday/Wednesday/Friday ($85) Tuesday/Thursday (or 2 days a week) ($65) 9AM-11AM 1:30PM-3:30PM preschool class at Lil’ Da Vinci Academy in the school year. I understand that my child may not be placed in the time slot of my preference based on the amount of children enrolled at Lil’ Da Vinci Academy. Every effort will be made to place your child in that time slot but can not be guaranteed.
