  3. VISION OF PLANNED CARE MODEL AT NPN • 50% of PCP’s with registry • 100+ PCP’s • 25% with more than one condition • Roll out of EMR with registry function

Increasing Registries • Recruitment of offices • Abstracting charts • Training of offices by QI team and Basic Training classes. • Coordinating electronic lab downloads. • Quest, Labs NW, PacLab • Coordinating faxing in of monthly summary reports. • Adding a second condition • Added choice of web based registry

  5. New Reports and Aggregating Data • Run charts for each provider – old and recent measures. Excel sheets linked. • Run charts for each provider with aggregate of whole practice • Run charts for each practice with aggregate of NPN. • Plan is to do aggregate of A1c, LDL and BP so as to compare with health plans.

  6. Facts, Tips and Lessons Learned • NPN offices are 1 to 10 providers in Pierce and South King county. • When new office brought on board frequent follow up is a must. (Every 2 weeks at least.) • Very difficult to implement registry in office with more than 3 providers if each one acts totally independent of the other.

  7. More Issues • Technological expertise • Data entry • Backing up dta file • People’s comfort zones • Staff turnover • Old equipment – hard drive, printers etc. • MS Access – 97 and 2000 versions. Especially interesting on one PC.

  8. More Issues • Don’t hassle the cost of MS Access if we have already spent countless man hours abstracting charts and inputting initial data. • IT contact outside of provider walls.
