  3. 0-25 Education, Health and Care plan (person-centred and outcome focused) Personalisation and budgets Much better relationships with young people and families Local offer and accountability Developing support for life outcomes Key Changes to the System set out in the Bill

Young will be able to request an assessment Colleges must use their ‘best endeavours’ for young people with SEN Colleges will need to co-operate with local authorities in exercising functions of the new system Admitting young people where the college is named in a plan Developing flexible packages Working closely with schools to support transition Following the new Code of Practice What the SEN Reforms Mean 16-25

Developing person-centred practice Developing strategic relationships with local authorities and partners Developing curriculum based on young people's aspirations for life Awareness of what works in terms of pathways into employment, independent living, friends, relationships and community and good health And in practice……..
