  5. Web based denominator data submission • Use your Citrix credentials to log in • Accessed from Health System Applications “Quality Management System”:

  11. Web based denominator data submission After submitting the data for your Unit, simply log out. For the first month, you will need to keep a record on paper as well as submitting it online. Beginning in June, you do not have to fax or email the denominator data forms – we will retrieve the information electronically.
