The RARE Campaign aims to reduce avoidable readmissions by 20% from the 2009 baseline by December 30, 2012. By enabling patients to spend an additional 16,000 nights at home rather than in the hospital, the initiative enhances care experience while saving an estimated $30 million for commercially insured patients. Currently, 82 hospitals and 75 community partners have prevented 2,607 readmissions, achieving a 13% reduction. The campaign continues to evolve, promoting data-driven strategies and collaborative efforts across the healthcare continuum.
