    1. HOW to APPLY If your client is new to SWEP

    2. STEP 1 Complete an Eligibility Form This is sometimes referred to as an Application Form. It is designed to provide information to the SWEP on whether your client is eligible for subsidy under the A&E program. The form requires a signature from your client or their guardian declaring their agreement to enquiries and understanding of the program. The form includes a declaration by a doctor that your client has a permanent disability http://swep.bhs.org.au/sites/default/files/forms/Application%202011_0_0.doc This form also includes information about your client’s next of kin and preferred language. If your client requires an interpreter, it should be noted on this form

    3. STEP 2 Complete a prescription form for each item of equipment you are requesting. Make sure you enter your SWEP registration number where requested, and that all parts of the script are completed. If you are submitting prescriptions for a number of equipment categories: You only need to fill in Items 1 to 3 on the Prescription forms ONCE. If you do this you MUST submit all forms relating to that client TOGETHER. You must complete ALL other sections of ALL the prescription forms. It cannot be assumed that “Relevant Characteristics”, “Clinical Justification” or “Implications for Non-Provision” will be the same for all equipment categories. If you need validation for your prescription, make sure you get the validating therapist to sign for the equipment category. Make sure you attach a quote for the item/s requested, or a supplier name and catalogue number on a separate page to the script.
