  4. Setting the Scene Dudley is split into 5 localities: Sedgley, Dudley North, Brierley Hill, Stourbridge and Halesowen. There is variation within and between wards in terms of deprivation. According to the 2001 Census Dudley has a population of 305,155 149,714 males & 155,441 females 1 PCT 1 MBC 0 Universities 0 Stadiums Merry Hill

Obesity in Dudley November 2004 - set up a multi-agency Obesity Task Group July 2005 launched the Dudley Obesity Strategy ‘Tackling Obesity – A Framework for Action in Dudley June 2006 – 1st weight loss service commissioned – Weight Watchers Oct 2007 – 1st MEND June 2008 - Weight Management Team established 2010 - 7 adult weight management services & 11 services for children

  13. NHS Dudley Weight Management Team (WMT ) Child Weight Management Referral Process Referral made by referrer (School Nurse. GP’s etc) Referral Form sent to WMT (fax, courier, email) Referral Form received; data inputted & family added to Services waiting list Receipt of Referral letter sent to family to include an overview of the WMT service’s (Within 10 working days of receiving the referral form) Family will be called (at the times/days specified on referral form) to discuss which Service is most appropriate for their family. 3 separate attempts will be made to contact the family. Telephone contact with family made No contact made A place on a suitable programme will be offered & confirmed. The child will be taken of the waiting list and placed onto the confirmed list. If family is unobtainable then a letter informing them that the WMT have tried to contact them will be sent. Families will be told a place has been reserved for them and will be asked to contact the WMT by a specified date to cancel or confirm. A confirmation letter will be sent to the family with details of venue, times and dates etc WMT will inform the referrer the programme the family has selected If the families do not contact the WMT they will be sent another letter informing them that will be taken of the waiting lists. This letter will give the family a deadline to contact the WMT to reserve a place for their family Family completes programme (75% attendance) Drop out or DNA (6 month cooling of period) Status report sent to referrer & SHA Referrer informed that the family has declined the referral

  15. MEND In Dudley: * Mind & Nutrition sessions are led by a Dietitian * Exercise Leaders are Dudley Council employees * PCT Volunteers support both theory & exercise Venues – Dudley Council Leisure Centres Land & water based – pool sessions vital in relation to retention

  16. Graduate Sessions • Rotational basis across borough • Weekly • Exercise Only • Staffed by West Bromwich Albion FC • Currently under review • Well attended but only by a small cohort of dedicated regulars

  17. Follow-Ups • Appointments at 3, 6, 9, 12 & 24 months • Tried a varitey of different activities • WBA FC- 39 children attended from across all of our Services 17 from MEND

Lessons Learned Strong links with referrers – waiting list in operation Highly qualified, passionate & enthusiastic team Regular face to face contact with families for up to 2 years Graduate Programme Free of charge to & their siblings Consultation with families past & present Ever changing – despite being weekly West Bromwich Albion, The Albion Foundation – good selling point Commitment to support children to access mainstream sessions in their activity of choice
