  2. Who We Are... • Established in1992 • Small Registered Charity • Covering the East End of Glasgow • Supporting anyone over the age of 16 years with a disability • Current Services offered: • Private Care at Home • Creative Breaks (Short Breaks) • Dementia Day Care – Bealach House

  3. How we used to do things... • Mostly Social Work Referrals • Static Times • Task focused • Little choice/flexibility for clients • We chose the staff

  4. How we do things today... • Clients come directly to us • Clients make up their own rotas • Not tasked focused, more outcome focused • More choice & flexibility for clients and their carer’s • Clients chose their staff

  5. Case Study 1 A 44 year old man with Cerebral Palsy, living in his own flat. He has a care package with a budget of £52,00 per year. He wants to meet new people, be connected to the community and pursue his hobbies and interests.

  6. Case Study 2 A 74 year old lady with dementia & bipolar disorder who lives on her own with a budget of £16,500 per year. She wants to stay at home, maintain relationships, feel safe & secure and build her confidence.
