You have an Accommodated Exam Request form available for completion One of your students has submitted an Accommodated Exam Request form.  Please login to complete your portion of the form.  Once completed, this information will be sent to the Office of Disability Services.  Thank you for your assistance with this process. Click Here When you receive this email, click the link above. You will then be directed to our online accommodated exam request application. Follow these next few steps to provide the Office Of Disability Services with important information regarding the administration of your exam. THANK YOU!

Reminders… • Emails will be sent to your University email account informing you of needed approvals. • Click on the link provided which states “The Office of Disability Services Accommodated Exam Request Web Site” • Once a form is submitted - revisions, updates, additions can only be made by the Office of Disability Services. • Send requests to MUDSExams@missouri.edu providing the course, instructor, original exam dates/times, updated dates and time, and any additional information. • Final Exams during the week designated for Final Exams will only be administered at 7:30am, 10:00am, 12:30pm, 3:00pm, 5:30pm and 8:00pm Monday through Thursday. Friday is reserved for make-up exams. These are all times set by the Registrar’s Office. • Exams will be returned to you at 8:00am, 12:00noon, or 3:00pm. Exams begun/completed after 3:00pm will be delivered on the next business day.
