Time with the Health Visitor may • require you to dress smartly in your own clothes. • Absence • Please notify the health centre if you are unable to report • for duty on 01383 830203. • Please also remember to contact the university on • 01382 388194 to let them know of your absence.

Welcome to Kelty Health Centre • Kelty Medical Practice has five GP partners and one GP SD. • The surgery covers the Kelty area, Kingseat & Cowdenbeath. • The team at Kelty also consists of a Triage Nurse • Two Nurse Practitioners, One Practice Nurse, Phlebotomists, • District Nurses, Health Visitors, a Practice Manager and a team of clerical and admin support staff. • Other members of the multidisciplinary team who provide • sessions in the surgery include School Nurses, Clinical Psychologists, Midwives, Podiatrists, Dietician, Smoking Cessation Specialist. Counselling services also provided. • Together this forms a primary health care team whose job is not only to treat illness as it arises but, where possible, to prevent illness through health education and support. • We hope you enjoy your placement at Kelty Health Centre.

Nursing Philosophy of Care • It is the aim of the Community Nursing Team at Kelty Health Centre to provide high quality patient centred care to all housebound patients by utilising a holistic approach to care within the community. • This is to maximise the health potential of not only the individual patient, but also address the wider health and social care needs of various client groups and carers. • All members of the Community Nursing Team shall endeavour to work together and alongside all members of the multidisciplinary team in a respectful, supportive way to ensure the creation and maintenance of a professional safe working environment.

Placement Objectives • To provide every opportunity for advancement of student learning • including developing core nursing skills and focusing on • providing learning experiences centred on the students particular • requirements. • To provide support and assistance in the development of student • non clinical skills such as communication and reflection. • To provide a more holistic learning experience through providing • opportunities to spend time with the multi professional team. • To provide opportunities for student to access and utilise all • relevant local and national guidelines and standards that • are required to ensure delivery of high quality evidence based care. • To ensure all students feel welcome and fully supported throughout • their learning experience and to feel part of the team. • To ensure students are supported not only by their mentor but also • by every other member of the team who are all dedicated to the • role of developing students understanding and skills.

Introduction to the Role of Community Nursing • Kelty Health Centre provides all service users with holistic, patient centred care to ensure all health requirements and expectations are met. • The Community Nursing Team at Kelty are an integral part of this care delivery service. • To ensure all housebound patients receive the most efficient and effective care, the Community Nursing Team strive to… • Provide all patients with care that is not only effective but also evidenced based and safe. • Continually involve all members of the multidisciplinary team to ensure all patients receive holistic, patient focused care. • Ensure all patients are treated fairly and with respect, ensuring patient confidentiality is respected and never compromised. • Maintain and enhance all relevant clinical skills alongside their own professional development. • Involve all patients, their families and carers in all clinical decision • making to ensure their care requirements are met. • Reduce the incidence of admission and re-admission by supporting and educating patients and the families on all relevant health issues.

The Multidisciplinary Team • During this placement you will have the valuable opportunity to see how every patient’s holistic care requirements are supported by spending quality time with various members of the multidisciplinary • team at Kelty. • There will be opportunities to spend time with a GP, Midwife, Podiatrist, Smoking Cessation Specialist and Specialist Cardiac Nurse alongside many other supporting roles that ensure every aspect of the patients’ health needs are provided for. • Spending quality time with these vital services will provide you with a more in-depth, richer understanding of the various roles and contributions these services make to improving the health outcomes of every patient seen at Kelty Health Centre.

On arrival to Kelty Health Centre! • On the first day of your placement you can expect to be… • Introduced to your mentor/ associate mentor • Oriented to the surgery including emergency procedures • Introduced to the Community Nursing Team and other members of the Multidisciplinary Team. • You will also spend time with your mentor/ associate mentor and… • Set your learning objectives for this placement in line with your student booklet. • Discuss and plan any specific learning goals you wish to • accomplish whilst on this placement. • Be provided with a diary planner for you to plan your learning. • Agree a date for your midway and final assessments.
