  2. Complete all fields then click Request.

  3. YOU DO NOT NEED A CERTIFICATION PIN TO SUBMIT A CRTK SURVEY!

  4. SEARCH BY FACILITY ID OR FACILITY NAME, NOT BOTH!

  5. Click here to access the Right to Know Survey for each specific facility

  6. Printable instructions for Web Application Click “Continue” to proceed

  7. Reporting Year Click on Company Info Button to Continue

  8. All fields will be pre-populated from previous submission, paper or electronic. Review and make changes as necessary. Note: You cannot make changes to the Facility Location. Click “REQUEST CHANGE” button to initiate these changes.

  9. Make sure to indicate below the reason for the change Enter changes as needed then click “Submit Your Request”

  10. If you get a new FACILITY ID based on your request, you will need to modify your “User Profile” to access your Facility information.All other changes do not require a change in your User Profile.

  11. YOU MUST ANSWER YES OR NO TO QUESTIONS B 1 AND B2. IF YOU ANSWER NO TO EITHER QUESTION YOU DO NOT NEED TO COMPLETE PART 2, THE SUBSTANCE LIST SECTION (UNLESS YOU ARE REPORTING SUBSTANCES PURSUANT TO EPCRA.)

  12. Once the Company Info Section is complete, click on “Save To File” and then click on “Substance List” to go to Part 2 of the Survey.

  13. Substances from previous submittal, paper or electronic, will be listed. All fields, except Max and Avg Daily Inventory and Days on Site, will be pre-populated from previous submittal, paper or electronic.

  14. All of fields, except Substance Number and DOT Number must be completed or you will receive an error message when you “Validate Chemical” Note:Container Description only needs to be filled out if “Other” is selected in Container Type

  15. Click to add another substance

  16. Select substances, then click here All substances in red are classified as EPCRA Section 302 extremely hazardous substances

  17. = Completed Substance = Not Completed Substance

  18. When finished saving all substances, click, “Verify Data” Status now complete When finished entering Info, click “Save to File”

  19. Click “Submit Survey”

  20. This page will need to be revised. Appendices 1 and 2 no longer exist. Click ACCEPT AND SUBMIT to submit the survey

  21. YOU MUST PRINT A COPY OF THE SURVEY TO SEND TO THE LOCAL AGENCIES Click ‘Exit’ to leave program Coordinator This page will need to be revised. Appendices 1 and 2 no longer exist. This means your survey was successfully submitted
