  3. Accessing EzVAC • EzVAC can be accessed a number of ways • Eclipsys patient record • WebCIS patient record • WebCIS home page • url

  4. EzVAC and Eclipsys

  5. To access EzVAC from Eclipsys. Once in a patient chart, click on EzVAC under the Tools menu…

  6. It will bring you directly into the patient’s EzVAC chart. You do not need to log in or enter the MRN again

  8. To access EzVAC from WebCIS. Once in a patient chart, click on the EzVAC link…

  9. It will bring you directly into the patient’s EzVAC chart. You do NOT need to log in or enter the MRN again

  11. Click on the “Immunization” link, this will bring you to the EzVAC home page

  12. You can log in using your WebCIS/Eclipsys password

  13. You can also get to EzVAC from directly from the web • EzVAC url: https://ezvac.cpmc.columbia.edu/ezvac • You need to be on a hospital or university computer or be connected through VPN

  14. Getting started in EzVAC…

  15. Selecting a Facility Code • The first thing that you need to do if you have never used EzVAC before is to select the facility that you are working from

  16. Click on the “Facility” link

  17. Scroll down and click on the facility that you are currently at…

  18. You will now automatically be logged into that facility whenever you log into EzVAC. • This is important for when you are entering immunizations. • You can look up immunizations of any child in the system, regardless of what facility you are logged into.

  19. You can make sure that you are logged into the facility that you are currently at

  20. If you are at more than one facility, you should make sure to change your facility code to the new facility

  21. If you have changed to more than one facility, every time you log in after that, EzVAC will ask you which facility you are currently working at that day… Just click on the link to the site you are currently at, and you will be logged into that facility

  22. To look up a patient’s record, click on “Record Number” to look up by “MRN”, or on Name to look by name or partial name, and then enter the information

  23. To look up a patient’s immunization record: click on the “Immunizations” link…

  24. The “short view” of immunizations gives you the name of the immunization and the date it was given

  25. The “long view” of immunizations gives you the full information for the immunization given. It also allows you to make changes to immunizations that were entered erroneously.

  26. To access school, daycare, camp and WIC forms: click on the Forms link…

  27. You can then click on WIC, camp, daycare or school form and the patients sociodemographic and vaccine information will be filled in automatically

  28. To access Vaccine Information Sheets (VIS), click on the VIS statements link…

  29. To enter new vaccines: Click on “New Vaccines/shots” link

  30. Click vaccine you want to enter…

  31. Use the pull down menus to make sure that vaccine information is correct

  32. When you are finished, scroll down to the bottom of the page and click “done” of you have no more vaccines to enter, or “more” if you have more to enter

  33. To enter old vaccines that were given elsewhere: Click on “old vaccine/shots” link

  34. Enter the date that vaccines were given and click the box next to all the vaccines given on that same date

  35. If you are not sure if a vaccine is already in EzVAC, look at the full list of vaccines already in EzVAC

  36. When you are finished, scroll down to the bottom of the page and click “done” of you have no more vaccines to enter, or “more” if you have more to enter

  37. Viewing immunizations directly • You can now view records directly in WebCIS and the WebCIS tab in Eclipsys without having to leave the electronic health record and enter into EzVAC

  38. You can see the immunization record directly from Eclipsys by using WebCIS tab. Click on

  39. Click on the Immunization link

  40. The patient’s immunization record will be displayedwithin the same screen. Scroll down to see the rest of the record.

  42. The patient’s immunization record will be displayed within the same screen. Scroll down to see the rest of the record

  43. Now, you can also do the Vaccine Adverse Event Reporting (VAERS) directly from EzVAC by using the provided link. Click on Forms

  44. Click on the Vaccine Adverse Event Reporting System (VAERS) link

  45. A new screen will pop up with the VAERS form. The patient’s demographic information is automatically filled out. Also, included are the instructions on how to submit the form. See arrows.
