A positive way to improve your health: • Personal Health Assessment • Earn up to 450 points by participating in the campus health fair. • My Health Assistant • Earn up to 1,300 points by setting, making progress toward and achieving health goals with the online Health Assistant. • Health Trackers • Earn up to 250 points by utilizing Health Trackers.

  3. And get rewarded! As you work toward your wellness goals, you’ll earn points redeemable for gift cards! You can redeem your reward certificates for gift cards to a variety of restaurants and retailers. *3rd reward is a special incentive specific to each college.

  4. How do I get started? Step 1: Visit floridablue.com and register or login. All you need is your Florida Blue member ID number (located on your member ID card) and a valid email address to register. Login

  7. Get involved … take your Personal Health Assessment (PHA) It’s quick, easy and confidential! • Earn up to 450 points when you take your PHA with biometric screenings at the campus health fair. Earn 50 pointsfor being a tobacco non-user. • Earn 50 points for completing your PHA online at floridablue.com. • Get a snapshot of your overall health. • See what you’re doing right and where you might have potential to improve your health. Reshape your health habits with My Health Assistant There are six areas to choose from. You decide which one you want to work on first: • Exercise • Maintain a Positive Mood • Nutrition • Quit Tobacco • Stress • Weight Loss Earn 50 points for each goal you set, 150 pointsfor each goal you achieve and up to 100 points for making progress toward and completing your weekly plan.

Adopt a Healthy Lifestyle with Health Trackers • Earn up to 50 points when you utilize the BLOOD PRESSURE Tracker. • Earn up to 50 points when you utilize the CHOLESTEROL Tracker. • Earn up to 50 points when you utilize the EXERCISE Tracker. • Earn up to 50 points when you utilize the STRESS Tracker. • Earn up to 50 points when you utilize the WIGHT Tracker. Earning points is easy!

  10. Frequently Asked Questions Who can participate in BlueRewards?If you are an employee, over age 18 and covered by Florida Blue through your employer, we encourage you to participate. How do I earn points? You can earn points by completing any of the activities listed on the Earn BlueRewards page on floridablue.com. For example, when you take your Personal Health Assessment at the health fair, you’ll earn 250 points! *Activities page screenshot is a sample and may not be specific to your program.

  11. Frequently Asked Questions (continued) When does BlueRewards start and end? You can earn points from February 1, 2013 through December 31, 2013. You can redeem your rewardsfrom February 1, 2013 through January 15, 2014. (If you completed a Personal Health Assessment at a health fair in January 2013, points will be awarded for your 2013 BlueRewards program.) How do I track my points? As you complete activities, you can track your progress on the Earn BlueRewardspage. You’ll see the points you've earned and your redemption status. Activities completed through WebMD tools will be reported immediately. Please allow four weeks for reporting of Florida Blue activities. All group-reported activities will be available the following month after the activity is completed. How do I redeem my reward? Once 250 points are earned, you will automatically qualify for the reward up to three times.

From the Earn BlueRewards page on floridablue.com, scroll through the activities and pick which activity you want to accomplish first. If I have questions, who should I ask?If you have a question about BlueRewards, please speak with a Care Consultant at 1-888-476-2227. If you have a benefit or claim question, please call the toll-free number on the back of your member ID card. How long will it take to obtain my certificate? Hallmark will send a reward certificate email to you with instructions for selecting your gift card and for providing your mailing address. Hallmark will send the gift card first via US Postal Service for receipt within 5-7 days.
