Guidelines to correct method of permit application • Access the correct application form on the website (www.gov.za; Departments; Health; Documents; Forms; Permit applications) • Fill the form with correct information : • Applicant details with physical address;Tel & Fax numbers • Product to be imported /Exported • Period required for Importing /Exporting • Details of who & where the product is exported to or Imported from. • Reasons for importing /exporting

Guideline to permit issue process • Application is received by Mr Setsetse • Permit is process by Mr Setsetse • Permit is submitted to Mr JR Mokonoto for checking and verification • Permit is submitted to CM for signature • Signed permits are Faxed to clients • Copies are made for filing • Original are posted to the clients
