  2. All Reading and Math Data Forms • Columns A through C are the same for all reading data forms • Columns A through C are the same for math EOG data, but changes for number knowledge A B C

  3. Math – Number Knowledge • For Math Number Knowledge ONLY, Column C changes to Student Birthdate. • Other columns shift one to the right, so School ID is in Column D, Teacher in Column E, etc. • Enter as mm/dd/yyyy A B C D

  4. All Reading and Math Data Forms • 3 digits only for regular public schools • Charter schools include 1 letter & numbers A B C
