  4. Our Mission The mission of GOCF is to empower Georgia communities to achieve improved and sustainable outcomes by working in a systems of care framework, which will ensure that all children and families are educated, healthy, safe, and growing.

To improve child and family well-being by developing productive programs that positively impact GOCF's results areas and associated outcome indicators, and  offer prevention, educational, and social service activities that are targeted to a specific population (See page 3 of RFP) • Purpose of the Request for Proposal (RFP)

  6. System of care is a philosophy that builds a framework to incorporate a broad array of services and supports that are arranged into an organized network. These networks focus on the whole needs of the child and family, coordinating prevention, intervention, and treatment services. • For more detailed information regarding System of Care, visit www.children.ga.gov and access the System of Care Toolkit (See pages 3-7 of RFP) • System of Care

  7. Strategic Results Framework Through a community-based system of care, the Governor’s Office for Children and Families offers grant funding for prevention and intervention services for children, youth, and families to assure they are educated, healthy, safe, and growing.

  10. The GOCF System of Care is designed to help communities coordinate and integrate their child prevention, early intervention and intervention programs in three areas: • Child Abuse and Neglect Prevention • Juvenile Justice • Youth Development (See pages 15-25 of RFP) • Program Areas

Child Abuse and Neglect Prevention Projected Available Funds: $500,000 Maximum Grant Award: $175,000 Number of Expected Awards: 3 Child Abuse and Neglect Prevention System of Care Strategic Results Framework   Applicants must select: ONE of the Result Areas (EDUCATED, HEALTHY, SAFE or GROWING) and ONEtarget population and Associated GOCF outcome measures (See pages 15-17 of RFP) • Program Area 1: Child Abuse and Neglect Prevention

  12. Juvenile Justice: Projected Available Funds: $679,000 Maximum Grant Award: $175,000 Number of Expected Awards: 4 (See pages 17-22 of RFP) • Program Area 2: Juvenile Justice

Applicants will need to select ONE Juvenile Justice focus area: Community Outreach for High-Risk Youth (Emphasis on Prevention and Early Intervention) OR  Community Services for Juvenile System Youth (Emphasis on Early Intervention & Intensive) Juvenile Justice System of Care Strategic Results Framework Applicants applying for juvenile justice funds must select: ONEof the Result Areas: EDUCATED or SAFE. ONE target population (Elementary, Middle, or High School Age) and Associated GOCF outcome measures. (See pages 17-22 of RFP) • Program Area 2: Juvenile Justice

  14. Youth Development: Projected Available Funds: $270,800 Maximum Grant Award: $175,000 Number of Expected Awards: 1 or 2 (See pages 22-25 of RFP) • Program Area 3: Youth Development

Youth Development Focus Areas Applicants must choose one area of focus: Abstinence Education Alcohol, Tobacco, and Other Drug Use Prevention Youth Employment Teen Mental Health Youth Development System of Care Strategic Results Framework  Applicants applying for youth development funds must select: ONEof the Result Areas: HEALTHY or GROWING. ONE target population (Elementary, Middle, or High School Age) and Associated GOCF outcome measures. (See pages 22-25 of RFP) • Program Area 3: Youth Development

Applicants must be: • A local unit of government or 501(c)(3) non-profit • An existing, emerging or new system of care site • Able to successfully provide quality SOC services and meet fiscal responsibilities • Able to submit timely and accurate program and financial reports • Award Eligibility

  17. Maximum Grant Award is $175,000 or Less • Fifteen month first year award • Three month planning period – up to $15,000 • 12-month Operating Budget – up to $160,000 • October 1, 2011 to December 31, 2012 • Grant Award Amount and Period

  18. Applicants may apply for two additional years of funding. Continuation funding is contingent upon: • Organizational capacity; • Performance history and grant compliance; • Availability of GOCF funds; and • Applicant’s ability to leverage resources and to support the System of Care in Years 2 and 3 and after the end of GOCF funding. • Continuation Funding

  19. Grant Award Year 2: The applicant will be eligible to apply for 100% of the base budget (up to $160,000) of the Year 1 award amount. The SOC will be required to secure leveraged resources equal to 10% of the Year 2 grant amount to directly fund or support the SOC. • Grant Award Year 3: The applicant will be eligible to apply for 100% of base budget of the Year 2 award amount. The SOC will be required to 1) secure leveraged resources equal to 20% of the Year 3 award amount to directly fund or support the SOC and 2) secure 20% cash match of the Year 3 award amount to directly fund the SOC after GOCF funding ends. (See page 28 of the RFP) • Sustainability Requirements

Leveraged funds may include in-kind and/or cash match. • Leveraged funds are realized when funds or cost-savings are secured for future SOC outlays from sources such as the award of public and private grants, cost-savings by re-directing services and supports to community programs and supports; governmental system change to permit SOC funding across agencies. • Leveraging - Examples

Leveraged funds may include in-kind and/or cash match. • Cash match is realized when funds are spent to pay SOC outlays, such as public and private grant funds, community cash donations, SOC community partner operating budgets that pays for SOC staff/contractors, operating costs and program services and supports. • Cash Match - Examples

Leveraged funds may include in-kind and/or cash match. • In-kind match is realized when supports and services are provided at no cost to the SOC partners such as provisions for food banks; baby and child supplies and clothing; refreshments for meetings; no cost use of office space, office equipment, IT equipment; use of recreational centers and libraries for service delivery, SOC participants providing child care during parenting meetings, family members and/or youth participation, SOC collaborative volunteer hours. • In-kind Match - Examples

  23. Grant Management System: No later than 30 days after the end of each quarter, the grantee will report program and participant data through the GOCF grants management system (GMS). GMS is an online data management and reporting system. • Evaluation: GOCF has hired an evaluation consultant to conduct a cross-site evaluation of grantees. Grantees will have the following roles and responsibilities within the cross-site evaluation: • Participate in and assist the cross-site evaluator with scheduling or administering interviews, focus groups, and/or surveys of stakeholders; and • Collect, enter, and maintain participant-level implementation and outcome data within the GMS data management system. (See page 29 of the RFP) • Evaluation & Reporting Requirements

  24. Any application postmarked after the June 6, 2011 deadline will be regarded as late and will not be considered for funding; • Applicant is not a public government entity or 501(c)(3) non-profit organization; • Implementation plan is not based on one of the result areas Educated, Healthy, Safe, or Growing; • Implementation plan is not targeted to one result area, one program area and one targetpopulation; • Implementation plan is unrelated to the areas of child maltreatment prevention and intervention, delinquency prevention, and youth development; • Failure to include application narrative, application budget, and application attachments; • Failure to provide appropriate number of copies and format requested; • Submission fails to include an original signature of the applicant’s executive officer or applicant’s fiscal agent on the application face sheet; • Application lacks original signatures where appropriate; or • Applicant is an individual agency not applying on behalf of a community collaborative, coalition, or some other community-based group intended to implement a SOC. (See page 29 of RFP) • Disqualification Factors

No more than six (6) pages • Clearly describe the SOC operations and process from outreach and referral to participants’ exit from the services. • Discuss the authority and roles of the partners and how the services will be seamlessly provided to participants. • Complete Form B: Strategy Implementation Plan • System of Care Methods and Procedures

No more than two (2) pages • Data collection methods, sources and roles and responsibilities. • What do you hope to learn from the evaluation and include the questions you plan to answer? • How were the local partners and stakeholders included in the evaluation planning process? • How do you plan to use the data to support sustainability? • Outcome Evaluation
