  3. How to “Be” Today • Focus on aligning with the goals of the Redesigned ESRD Network Contract – AIM 1 Domains 4 and 5 • Actively Engage in Hearing and Sharing thoughts and creativity,and new ideas • Preparing to implement at least one idea or action gleaned from this presentation.

  5. Vascular Access Management • Improve AV Fistula Rates for Prevalent Patients • Reduce Catheter Rates • Support Facility Reporting • Spread Best Practices • Provide Technical Support • Recommend Sanctions

  7. Domain 5 Patient Safety Healthcare Acquired Infections:

Patient Safety Healthcare- Acquired Infections (HAI’s) Support National Healthcare Safety Network (NHSN) Enrollment and Reporting • 90% of Dialysis Facilities Enrolled (Monthly Goals) • 75% of the monthly goal for enrollment achieved • 80% of Facilities Reporting for at least 6-months (Monthly Goals) • 75% of the Monthly Goals for reporting achieved • Provide education and support with the enrollment process • Verify accuracy and timeliness of facility data in the NHSN database • Group Administrator Role • Conduct QIA – based on trending and tracking of collected data

Establish HAI LAN – Establish – Collaborate – or Both One Network’s Perspective • Join your States’ QIO HAI LAN • Collaborate with other healthcare providers • Invite Dialysis Leaders to join QIO LAN • Develop your own HAI LAN • Seek participation of healthcare providers working on HAI or CLABSIs • Rapid cycle improvement…planning and implementation

  10. Reduce Rates of Dialysis Facility Event • QIA – Implementation based on data from NHSN • Involve 20% of facilities and at least 5% of the Network patient population • Demonstrate a 5% relative Improvement Rate on the Dialysis Facility Primary Measure – Report Monthly
