Een beetje suf…. Dhr G 68 jaar. Coloncarcinoom Hemicolectomie verricht Week later re-OK: necrose anastomose tgv ischemie; Hartman procedure verricht 2 weken later: septische shock. Bij laparotomie geen verklaring shock in buik gezien. Kweken afgenomen, start piperacilline-tazobactam.

  2. Dhr G 68 jaar • Coloncarcinoom • Hemicolectomie verricht • Week later re-OK: necrose anastomose tgv ischemie; Hartman procedure verricht • 2 weken later: septische shock. Bij laparotomie geen verklaring shock in buik gezien. • Kweken afgenomen, start piperacilline-tazobactam

  3. Vervolg casus • BK 1 flesje positief

  4. Vervolg casus • Volgende dag: inmiddels BK 3 flesjes positief • Gram-positieve staven

  5. Vervolg casus • Patiënt nog koorts tot 40° • Erg suf voor de kortdurende anesthesie die gegeven is tijdens laparotomie

  6. Bloedkweek • Listeria monocytogenes!

  7. Hypothese • Nosocomiale infectie onwaarschijnlijk • Gastro-intestinaal dragerschap L. monocytogenes • Weerstandsvermindering door onderliggend lijden en operaties • L. monocytogenes wordt invasief • Sepsis + meningitis

Therapie • Piperacilline en Listeria • Casuïstiek; meestal piperacilline + aminoglycoside • In-vitro studie naar PBP affiniteit voor antibiotica;hoge affiniteit voor amoxicilline > piperacilline > imipenem Penicillin binding proteins in Listeria monocytogenes. Achtergrond informatie • Iedere bacterie heeft 4-8 PBP’s =penicilline bindend proteïne; is eiwit betrokken bij celwand synthese • PBP 1a en PBP 1b; Transpeptidase enzyme dat cross-linking van peptidoglycaan ketens katalyseert • Beta-lactam antibiotica zijn “suicide inhibitors” van transpeptidase

Achtergrond informatie • Werking β-lactam antibioticum bij Gram-positieve bacteriën: binding aan PBP • Werking β-lactam antibioticum bij negatieve bacteriën: moet eerst door porine heen, dan binding aan PBP • Resistentie: PBP met lage affiniteit, bacterie vormt β-lactamase, porine is te klein voor passage antibioticum

  11. Achtergrond informatie • β-lactam antibioticum bindt met β-lactam ring aan PBP waardoor PBP onwerkzaam wordt • Uitval PBP1: osmotische lysis cel • Uitval PBP2: staafvorm gaat verloren; bacterie wordt rond • Uitval PBP3: septumformatie gaat verloren; filamentvorming treedt op

  12. Vervolg casus • start amoxicilline 6dd 1 gram naast piperacilline-tazobactam • Blijft wat suf • Na paar dagen alsnog LP: geen groei • Therapieduur 3-6 weken
