LEARNING OBJECTIVE آهداف آموزشی AT THE END OF THIS LESSON YOU WILL BE ABLE TO: در ختم این درس مداومین قادر به آموزش موضوعا ت ذیل خواهند شد : ACTION: Provide Basic Emergency Medical Care for an Amputation فعالیت: فراهم کردن مراقبت های اساسی عاجل طبی برای امپوتیشن . CONDITIONS: A casualty has experienced a partial or complete amputation. All other more serious injuries have been assessed and treated. You will need a tourniquet, marker, sterile gauze, intravenous (IV) equipment, saline, plastic bag, container, ANA Form 1380 (Field Medical Card), ice, and cravats. شرایط: مصدوم قطع قسمی یا تام عضوء را تجربه نموده . جروحات جدی دیګر ارزیابی و تداوی ګردیده است . شما به تورنیکت ، مارکر ،ګاز معقم ، لوزام برای زرقیات داخل وریدی ، سیروم سالین، خریطه پلاستیکی ، فورم ۱۳۸۰ اردوملی یا کارت صحی ساحویی، یخ ، و کروات ضرورت دارید. STANDARDS: Provide correct treatment based on severity and location of amputation without causing further injury to the casualty معیارات : به اساس وخامت و موقعیت امپوتیشن مراقبت صحی درست را بدون انکه مصدوم بیشتر متضرر ګردد فراهم نماید.

  3. Determine the Type of Amputation - An amputation (severing) of a limb is handled somewhat differently than a heavily bleeding wound on a limb. The amputation can be complete or partial. The amputation can be of a limb or a part of the hand or foot. • نوع امپوتیشن ر اتعین نماید- قطع عضوء (جداشدن ) عضو‌‌ء قدری از خونریزی شدید جرحه در یک نهایت که بادست احساس میشود متفاوت است. امپوتیشن میتواند که قسمی یا تام باشد. امپوتیشن میتواند یک عضو‌ء کامل یا قسمتی از دست یا پاه باشد. • Complete Amputation - In a complete amputation, the part of the limb below the amputation site is completely severed (cut off). • قطع تام- در قطع تام ، قسمتی از عضو پاینتر از محل امپوتیشن بشکل تام جدا شده یا قطع شده میباشد. • Partial Amputation - In a partial (incomplete) amputation, the portion of the limb below the wound (site of the incomplete amputation) is almost completely severed from the body, but some skin tissue continues to connect the portion of the limb below the wound to the rest of the body. • قطع قسمی- در قطع قسمی ( ناتام ) ، قسمتی از عضوء پاینتر از محل جرحه ( محل پاینتر از قطع قسمی ) تقریباٌ بشکل مکمل از عضویت جدا ګردیده است، مګر بعضی از قسمت های انساج جلدی باز هم به ناحیه پاینتر از جرحه با عضویت در تماس میباشد. PROVIDE BASIC EMERGENCY MEDICAL CARE FOR AN AMPUTATIONفراهم کردن مراقبت های اساسی عاجل طبی برای امپوتیشن Amputation of a Limb - The amputation of a limb exists when the amputation site is on the upper arm, elbow, forearm, wrist, thigh, knee, lower leg, or ankle. قطع طرف - قطع طرف در برګیرنده قطع عضوء در محل بالاتر دست ، ارنج،بازو ،مچ دست، ران ، زانو،قسمتی پایینی پاه ، و زانو میباشد. Amputation of Part of a Hand or Foot - An amputation of a part of the hand exists when the amputation site is below the wrist and does not involve the entire hand. An amputation of a part of the foot exists when the amputation site is below the ankle and does not involve the entire foot. قطع قسمتی ازدست و یا پاه - قطع قسمی دست زمانیکه محل امپوتیشن پایینتر از بند دست بوده و دربرګیرنده تمام دست نباشد میباشد. امپوتیشن قسمی پاه زمانیکه قطع عضو‌‌‌‌ء پایینتر از بجلک پاه بوده و در برګیرنده تمام پاه نباشد میباشد.

  5. Performance Steps • قدمه های عملی • 1. Assess the casualty. (Task 081-86A-0155: Perform Trauma Casualty Assessment) • ۱.. رزیابی مصدوم .( درس ۰۸۱-۸۳۳-۰۱۵۵ اجرایی ارزیابی مصدومین تروما) • 2. Treat immediate life threats as they are discovered. • ۲. تهدیدات حیاتی را بمجردی دریافتن معالجه نماید. • NOTE: If an obvious amputation with significant hemorrhage, immediately apply a tourniquet. (Task 081-86A-0210: Apply a Tourniquet) • یادداشت: اګر امبپوتیشن واضح با خونریزی قابل توجه موجود باشد، فوری تورنیکت را تطبیق نماید( درس ۰۸۱-۸۳۳-۰۲۱۰ تطبیق تورنیکت) . Combat environment (under fire). • الف. درمحل یا میدان محاربه ( تحت آتش). • (1) Move the casualty to a covered and concealed location. • ۱) مصدوم را به یک محل که دارای سترو اخفا باشد انتقال دهید. • (2) Immediately apply a tourniquet. • ۲) فوری تورنیکت را تطبیق نماید.

Noncombat environment (not under fire). • ب.محیط غیر حربی( زمانیکه تحت مستقیم آتش نباشید). • (1) Implement airway, breathing, and circulation. • ۱) طروق تنفسی ، تنفس و دوران خون را ارزیابی نماید. • (2) Initially apply a tourniquet to control bleeding. • ۲) در شروع برای کنترول خونریزی تورنیکیت را تطبیق نماید. Perform emergency management for traumatic amputations. • ۳. تداوی عاجل را برای امپوتیشن تروماتیک اجرا نماید. • (3) The tourniquet may be loosened and bleeding should be controlled by • attempting other means. • ۳) ممکن که تورنیکیت سست ګردیده و خونریزی با ید به طروق دیګر کنترول ګردد. • (a) Direct pressure. • الف) فشار مستقیم. • (b) Pressure point. • ب) محلات فشاری . • (c) Pressure dressing. • ج) پانسمان فشاری . • . Perform emergency management for traumatic amputations. • ۳. تداوی عاجل را برای امپوتیشن تروماتیک اجرا نماید. • b. Noncombat environment (not under fire). • ب.محیط غیر حربی( زمانیکه تحت آتش مستقیم نباشید). • (4) Using a marker or the casualty's blood, make a "T" on the casualty's • forehead. • ۴) علامت تی را با استفاده از مارکر یا خون مصدوم بالایی پیشانی مصدوم نوشته نماید. • NOTE: With either of the above methods, consider the time to definitive care and the severity of blood loss. A tourniquet that prevents excessive blood loss and that will be removed within a few hours at a definitive care facility is preferable to excessive blood loss while trying other methods. • یادداشت : بایکی از میتود های فوق ، زمان و تداوی انجام شده و شدت خونریزی را یادداشت نماید. زمانی امتحان نمودن طروق دیګر خونریزی شدید متصوربوده ، تورنیکیت از خونریزی شدید جلوګیری نموده و در یک مرکز مجهز صحی در ظرف چندین ساعت دور ګردیده و ترجیح داده میشوده.

PROVIDE BASIC EMERGENCY MEDICAL CARE FOR AN AMPUTATION فراهم کردن مراقبت های اساسی عاجل طبی برای امپوتیشن The need for immediate access to a tourniquet in such situations makes it clear that all soldiers on combat missions have a suitable tourniquet, such as the Combat Application Tourniquet (CAT) shown above, readily available at a standard location on their battle gear and that soldiers be trained in its use. This tourniquet will also be found in the Afghan First Aid Kit (AFAK) and in the M5 Medical bag. ضرورت دسترسی فوری برای تورنیکیت در چنین شرایط این را واضح میسازد که تمام عساکر در ماموریت های دارای تورنیکیت مناسب میباشندمانند ترونیکیت حربی یا کت که شما تصویر ان را در بالا مشاهده مینماید، در محل معین اسباب حربی شان موجود بوده وسربازان در باره شیوه استفاده از ان تعلیم دیده اند. این تورنیکیت در بکس کمک های صحی اولیه و بکس کمک های صحی افغان قابل دریافت است.

  11. 4. Treat for shock. (Task 081-86A-0047: Initiate Treatment for Hypovolemic Shock) ۴. شاک را تداوی نماید.( درس ۰۸۱-۸۳۳-۰۰۴۷: اغاز تداوی برای شاک هایپو والمیک ) 5. Care for the amputated part. ۵. مراقبت از محل قطع شده. a. Rinse free of debris with saline if available. الف. اګر سالین در دسترس باشد محل را شستشو نماید . b. Wrap the part loosely in saline-moistened sterile gauze. ب. ساحه را با ګاز مرطوب شده با سالین بپیچانید. c. Seal the amputated part inside a plastic bag or wrap it in a cravat. The amputated part should then be placed in another container containing ice. Keep it cool, but do not allow it to freeze. ج. عضوء قطع شده را در یک خریطه پلاستیکی یا کروات بپیچانید. و عضوء قطع شده باید در یک بکس جداګانه که دارای یخ است نګهداری ګردد . تا سرد نګهداری ګردد، مګر انقدر سرد هم نه که عضوء قطع شده یخبندان ګردد. Avoid further injury to the amputated part. • د. از جراحت بیشتر عضوء قطع شده جلوګیری نماید. • (1) Never warm an amputated part. • ۱) ساحه قطع شده را هر ګز ګرم ننماید. • (2) Never place an amputated part directly in water or on ice.۲) عضوء قطع شده را هرګز مستقیماْ داخل اب یا یخ ننماید. • (3) Never use dry ice to cool an amputated part. • ۳) هر ګز از یخ برای سرد کردن عضوء قطع شده استفاده ننماید. • e. Transport the part with the casualty to the hospital for possible reimplantation or skin graft. • هـ . عضو قطع شده را برای ګرافت یا پیوند جلدی و یا نصب دوباره ان با مصدوم انتقال دهید.

  13. 6. Record all treatment on the casualty's FMC. • ۶. تمام تداوی اجرا شده را درکارت صحی ساحویی مصدوم یادداشت نماید. • 7. Evacuate the casualty to a definitive care facility as soon as possible. • ۷. هر قدر زود که ممکن است مصدوم را به مرکز صحی مجهز تخلیه نماید. Critical to this is evacuating the casualty to a definitive care facility as soon as possible. در دراز مدت نتیجه تداوی امپوتیشن نظربه فهم خوبتر از تداوی امپوتیشن تروماتیک ، مراقبت های اولیه و حیاتی ، تخنیک های جدید جراحی ، احیایی فوری ، و طرح جدید ساختن اعضایی مصنوعی ، بهبود یافته است . تخلیه مصدوم به مرکز صحی مجهز یا نهایی هر قدر زود که ممکن است حیاتی میباشد QUESTIONS سوالات ؟
