  2. Role of Radiotherapy • Rare tumors. • Retrospective studies. • Small number of patients. • Not Radiosensetive tumors.

Role of Radiotherapy ADJUVANT: • controversial. • ?? adjuvant treatment after complete resection for residual microscopic disease. • A 10-year crude survival rate of 37% has been reported after combined surgery and irradiation for functioning adrenal carcinomas. • However, in these tumors an earlier stage at diagnosis gives a better chance of complete removal.

  4. Role of Radiotherapy POSTOPERATIVE RT: • ?? Treatment of residual macroscopic disease and nodal. • Long-term objective tumor control.

  5. Role of Radiotherapy PALLIATION: • Inoperable: • Distant metastases e.g. Bone metastases. • Good response.

General Management Surgery • The primary treatment for adrenocortical carcinoma. • Curative surgery is feasible in only a few patients because of the size of tumor at the time of diagnosis. • However, complete resection is the only treatment that offers long-term disease-free survival. • Surgery can be used as a palliative procedure in patients with incurable functional tumors; partial resections are attempted to improve endocrine manifestations that cannot be controlled medically and to relieve local symptoms. • Careful attention must be given to preoperative, intraoperative, and postoperative steroid replacement, because the contralateral adrenal gland is often atrophied.
