  3. Although the R-R interval is regular, the cause is atrial flutter with 2:1 conduction, with a “downstream” abnormality of typical RBBB. The computer believes that all rapid wide-complex rhythms represent vent. tachycardia.

  4. Let’ agree that the irregularity in the top tracing is due to atrial fibrillation and that the rhythm is not ventricular tachycardia. The QRS morphology after conversion ½ hour later is identical.
