Phoebe Putney Cancer CenterNutrition Services Goals 1) Patients complete treatment as well nourished as possible 2) Help resolve or lessen side effects through education Program details Two full-time oncology dietitians: • 1 RD - Heme/Onc: RD referral per screening and as requested by staff during treatment • 1 RD - Rad Onc: RD referral per screening; prior eating difficulties and “high risk” diagnosis/treatment

AnMed Health Cancer CenterNutrition Services Goals • Proactive, patient-centric care and education on nutrition • Address side effects and nutrition needs early to lower overall anxiety and reduce crisis situations Program details Multi-tiered approach: • RD available, but shared with other areas; focuses only on ‘high risk’ patients • Patient navigators play critical role; nutrition addressed proactively • Refer patients to the American Cancer Society’s Dietitian on Call to help fill gaps

AnMed Health Cancer CenterNutrition Services: Benefits • Able to cost-effectively serve more patients with nutrition needs through our navigators and referring to the American Cancer Society • With navigator involvement, able to address problems proactively and initiate nutrition-related interventions earlier • Navigators have time to address and follow-up, and specific goals around nutrition • Provide education—Cancer Care 101 to all new patients • Potentially better quality of life and treatment outcomes • Able to avoid dehydration, or halting therapy due to excessive weight loss, low blood counts

Dietitian on Call • Free, personalized telephone nutrition assistance: treatment side effect management, address, healthy lifestyle or weight loss after treatment completion • Credible nutrition information and resource from oncology dietitians for those without access to a dietitian • Available to patients or caregivers before, during , or after treatment
