Duties of Individual Being Graded Provide brag sheet • Complete brag sheet to provide information to your grader • Provide examples and be quantitative if possible Be familiar with the FITREP form IOT provide the appropriate information • Include if you are not in standards • Provide swim qual/PRT information

  8. Blocks 1-9 Block 1—Name of member: • Use last name followed by a comma, then first name and middle initial. Use all caps and no punctuation. Ex: BURDELL, GEORGE P Block 2—Grade/Rate: • Use class, grade, or rate that the member is at the end of the reporting period. For MIDN, use class. (Ex.: 1/C); for OCs, use OC; for MECEPs, use rank. Block 3—Designation: • Enlisted warfare/qualification designators. Blank for midshipmen. Use the abbreviated version. For OCs with two designators, separate them with a slash. Ex.: AW/SW Block 4—SSN: • Use school student ID number. Do not use social security number. Block 5—Duty/Competitive Status: • For midshipmen, mark INACT. For OCs/MECEPs, mark ACT. Block 6—UIC: • Unit code that midshipman is attached to: • Georgia Tech: 63301 • Morehouse: 68881 Block 7—Ship/Station: • Unit that midshipman is attached to: • Georgia Tech: NROTCUGIT • Morehouse: NROTCUMH Block 8—Promotion Status: • Mark REGULAR. Block 9—Date Reported: • In yrMONda format, put the date that the semester started when the midshipman, OC, or MECEP joined the unit.

  9. Blocks 10-21 Blocks 10-13—Occasion For Report: • Mark PERIODIC. Block 14-15—Period Of Report: • Use yrMONda format. Use first day of the semester and last day of the semester. ISSUE: To avoid software issue, after writing in the “TO” date, save fitrep with box still selected. This will keep NavFit from auto-correcting the date. Ex. From: 07AUG20, To: 07DEC14 Block 16—Not Observed Report: • Leave blank. Blocks 17-19—Type Of Report: • Mark REGULAR. Block 20—Physical Readiness: • Use drop down menu. PRT codes are as follows: P is passing, F is failing, and M is medically waived. Weight standards are as follows: WS is within standards, NS is not within standards, and MW is for medically waived (injury, medical condition, or pregnant). MIDN on remedial or those who fail the PRT should be noted in the comments. Block 21—Billet subcategory: • Write NA.

Blocks 34-36 Block 34: Unit participation, extracurricular activities, contribution to unit morale, recruitment, and personal attitude. 1: Does not participate, unhealthy attitude, and/or >3 UAs 2: Barely participates, sub-par attitude, and/or ≤3 UAs 3: Somewhat participates, average attitude, and/or ≤2 UAs 4: Participates in most activities, above average attitude, and/or ≤1 UA 5: Participates actively, exemplary attitude, and no UAs Block 35: PFA scores (first round: inventory, second round: official), and personal appearance (in and out of uniform). 1: Fail PFA / 174 or less on the Marine PFT and/or constant unsatisfactory appearance 2: Satisfactory PFA / between 175 -224 on Marine the PFT and/or frequently out of regulations for appearance 3: Good PFA / between 225 – 249 on Marine the PFT and/or always abides by appearance regulations 4: Excellent PFA / between 250 – 284 on Marine the PFT and/or always abides by appearance regulations 5: Outstanding PFA / 285 or higher on the Marine PFT and always abides by appearance regulations Block 36: Teamwork, mentoring abilities, and community service 1: Does not contribute to peer’s growth, does not assist with unit work, and/or failed to do any community service hours 2: Little contribution to peer’s growth, little contribution to unit work, and/or did not complete all community service requirements 3: Average contribution to peer growth, does only tasks that are asked, and/or met base community service requirements 4: Proactive in mentoring other midshipmen, proactive in finding work to do, and/or exceeded community service requirements 5: Inspires and encourages other midshipmen, is constantly helping with unit work, and greatly exceeded community service requirements

Blocks 37-39 Block 37: Knowledge of COD and pro-knowledge averages. 1: Unsatisfactory COD and/or quiz average <51% 2: Progressing COD and/or quiz average between 51-74% 3: Average COD and/or quiz average between 75-79% 4: Above average COD and/or quiz average between 80-89% 5: Exemplary COD and quiz average >89% Block 38: Leadership for 3/c and above and followership for 4/c. 1: Unable to perform in billet 2: Unsatisfactory performance in billet 3: Meets bare minimums in billet performance 4: Above average performance in billet 5: Exemplary performance in billet Block 39: Potential as a future officer. 1: Shows serious deficiency in military aptitude and may not make it through the program 2: Needs improvement in military aptitude and is struggling with the requirements of the program. 3: Adequate military aptitude and will make a competent officer 4: Excellent military aptitude and will make a great officer 5: Exemplary military aptitude and will be a top tier officer

  15. Blocks 40-41 Block 40—Billet Recommendation: • First block should be billet recommendation for the following semester, and the second block is for the semester after that. Use appropriate abbreviations. Ex: LCPO, DIVO Block 41—Comments on Performance: • Comments should be written during first and second round fitreps. Comments should be written in full sentences that are direct and to the point, and do not have to be in all caps. There should not be any information that is “filler.” Use 10pt font and use the entire space. Any 1.0s or 5.0s must be explained thoroughly. If midshipman is on remedial, it must be noted in the comments. First sentence should be an overall statement of performance. In following sentences, itemize positives and negatives and expound upon general performance of the person being evaluated. Last sentence wraps up the main points and addresses any last points that need to be made. The bottom portion of the fitrep should be formatted as follows: • GPA: 4.00 Rank: 1 of 20 PRT: GOOD • Naval Aptitude Average: 3.4 • *Naval Aptitude Average is calculated by averaging blocks 33 through 39.

Blocks 43-47 Block 42—Promotion Recommendation: • Leave blank for advisors to complete. • No more than 20% can receive early promotes (EP) and the sum of EPs and MPs cannot exceed 60% of all recommendations. • The following must be marked as significant problems (SP) if: (1) semester GPA is lower than 2.5, (2)PFA/PFT is failed by NROTC standards, or (3) the midshipman is on warning, probation, or leave of absence (LOA). Fitreps should be ranked by class for both first and second round. Block 43: • Compile all fitreps and write total in each section. Block 44—Reporting Senior Address: • Enter the following: • COMMANDING OFFICER • NAVAL ROTC ATLANTA REGION Block 45—Signature of Reporting Senior: • Leave Blank. Signed by permanent staff counselor. Block 46—Signature of Individual Counseled: • Signed after individual is counseled each round. Block 47—Typed name, grade, UIC and signature of Regular Reporting Senior: • Leave blank.
