Through the collaboration of medical professionals the clinic focuses on the health needs of the community and offers preventative care, dental care, general health care, and family care.”

Organizational Goals Goals • Ensure healthcare access for poverty level families in Kent County. • Provide health education to family members in order to promote a healthy lifestyle. • Assess and monitor the health of children and their families. • Create a trusting atmosphere that promotes accountability for healthy lifestyles. • Provide leadership in meeting the healthcare needs of the community.

  6. Organizational Objectives Engage in a focused and collaborative health plan for families on their first clinic visit. Educate our patients on the importance of preventative care. Monitor patient outcomes regularly and address patient concerns to ensure collaboration. Offer culturally competent care to poverty level families every visit Objectives

  8. Nursing care assistant/medical assistant • Responsibilities include: collecting blood work, managing specimens, obtaining vital signs or other tasks as delegated by the RN, LPN, PA, or NP. Other duties include medical supply inventory management and some housekeeping duties. Must be team-oriented, good with time management, and organized. It is expected that all employees provide ethical care, carry themselves professionally, collaborate respectfully, and honors patient privacy. • Qualifications Required: Current MA or NCA certificate in state of Michigan, phlebotomy experience preferred

Registered nurse/licensed practical nurse • Responsibilities include: Patient advocacy and basic medical preventative care as ordered by the Physician, Physician Assistant, or Nurse Practitioner including immunizations, reviewing the patient’s medical history, completing new patient admissions, and interdisciplinary collaboration with outpatient services, counseling, dieticians, basic need or other resources. It is expected that all employees provide ethical care, carry themselves professionally, collaborates respectfully, and honors patient privacy. • Qualifications Required: Current license to practice as RN or LPN in State of Michigan

Physician Assistant/nurse practitoner • Responsibilities include: Primary and preventative medical care include physical assessment, diagnosis, medication management, outpatient testing, and referral to specialists or emergency care. It is expected that all employees provide ethical care, carry themselves professionally, collaborate respectfully, and honors patient privacy. • Qualifications Required: Current License to Practice as a Physician Assistant, Family Nurse Practitioner, or Adult Nurse Practitioner in the State of Michigan

Dental hygienist • Responsibilities include: Basic dental cleaning and care, educating patients on home care and affordable dentists for needed dental work outside of hygienist’s scope of practice. It is expected that all employees provide ethical care, carry themselves professionally, collaborates respectfully, and honors patient privacy. • Qualifications required: License to practice as dental hygienist in State of Michigan

  12. physician • Responsibilities include: Physician is expected to physically visit the Kent County Community Clinic at least monthly to support midlevel practitioners. He or she should be available by phone as needed during business hours, with response to page within thirty minutes. The Physician should have a heart for the underserved, be dedicated to practicing ethically, and act as a mentor for midlevel practitioners. It is expected that all employees provide ethical care, carry themselves professionally, collaborate respectfully, and honor patient privacy. • Qualifications Required: M.D. or D.O. current in State of Michigan, experience with family practice or urgent care preferred

  13. Medical director • Responsibilities include: Director will supervise the administrative and patient services components of the Kent County Community Medical Clinic. He or she is responsible for human resources, hiring and dismissal of staff, and annual evaluations. Director should be invested in employees, as well as the clinic’s mission and philosophy statements, and should make every effort to meet the personal and organizational needs of the clinic. It is expected that all employees provide ethical care, carry themselves professionally, collaborate respectfully, and honor patient privacy, the director should lead by example. • Qualifications Required: Degree in business administration, experience with human resources and management

  14. Finance & accounting personnel • Responsibilities include: Managing employee payroll, clinic medical expenses, and patient payments as well as developing a budget. It is expected that all employees provide ethical care, carry themselves professionally, collaborates respectfully, and honors patient privacy. • Qualifications Required: Degree in business administration, accounting, or finance, experience preferred

Office professional • Responsibilities include: Answering phone calls with energy, completing and filing necessary paperwork for appointments, billing for patient visits, scheduling appointments, and organizing employee scheduling, as well as light housekeeping duties It is expected that all employees provide ethical care, carry themselves professionally, collaborates respectfully, and honors patient privacy. • Qualifications Preferred: Medical Terminology course or experience with medical billing
