  2. What is success? • A shift in focus to prevention • A movement of people who advocate for community health • Changes in laws, policies and practices that improve the health of all residents

  3. What we hope to achieve at the end of 2020 • Reductions in youth violence • Reverse the childhood obesity epidemic • Provide a health home for all children • Increase school attendance

  4. Sacramento BHC target area • Pop: 89,000 • 41% Latino • 21% Asian Pacific Islander • 19% White • 12% African American • 7% Other • 44 languages spoken in Sac City Unified School District

  5. What is the BHC Hub? • Coalition of residents, grantees and partners working towards building a healthier community by striving to reach the 4 big results

Sacramento BHC Full Ful Full Hub Collaborative: Central table of all interested stakeholders Hub Host Team: Supports work of hub through communication and collaboration support Steering Committee: Representatives of the full hub 5 adult residents 4 youth residents 8 organizations Organizations Systems Leaders • 1 FTE Hub Director • 1 FTE Hub Coordinator • 1 contract media creator • PT contracted Evaluation Learning Coordinator Youth Adult Residents Health Access Youth Development Healthy Land Use and Engagement Economic Development Food Access Workgroups: Moves forward work of BHC

  7. Reductions in youth violence Sacramento Safe Community Partnership Program (AKA Ceasefire) Community night walks in Mack Rd and Oak Park SCUSD adopts district-wide anti-bullying policy

  8. Reverse the childhood obesity epidemic • Running path at WCW constructed through partnership with Sac Kings • Implementation of SCUSD’s district-wide PE program • SCUSD’s Healthy Foods Task Force: salad bars in all schools • Healthy Development in Oak Park opposing McDonald’s • Community garden on private vacant lots ordinance • Oak Park Farmer’s Market accepts CalFresh

  9. Provide a health home for all children • Pilot community health navigator program partnering with SE Asian and Latinos communities • Partnering with Sierra Health Foundation on assessment of clinic capacity and strategic plan to prepare for 2014 • Working on a school based health center at Hiram Johnson High School opening in the Fall

  10. Increase school attendance • Pilot Men’s Leadership Academy in 3 High Schools • Building template on chronic absence data for each school within SCUSD • Youth conducting education campaign on zero tolerance school suspensions and expulsion policies • SCUSD’s Connect Center • Increase federal reimbursements to schools providing health related services to students
