County Health Rankings • County Health Rankings is a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute • It’s a “call to action” for communities to work together to develop programs and policies that help people lead healthier lives • Each County in the State is ranked according to it’s health outcomes (mortality and morbidity) and health factors (health behaviors, clinical care, social and economic, and the physical environment)

  3. Highlight: San Bernardino County • Robert Wood Johnson Foundation developed a nine minute video featuring how the Department’s Healthy Communities Program is partnering with local cities to affect change • Celebrate your community doing well……..

  4. Current Ranking • 2012 County Health Rankings were released on April 3, 2012 • How did San Bernardino County rank? • 2010: 45 out of 56 counties • 2011: 44 out of 56 counties • 2012: 41 out of 56 counties (most improved County in CA)

  5. Other Department Collaborations • Community Vital Signs (CVS) • San Bernardino County Reentry Collaborative (SBCRC) • Federally Qualified Health Centers (FQHC)

Community Vital Signs (CVS) • Community Vital Signs initially started as a partnership between the Departments of Public Health and Behavioral Health, as well as Arrowhead Regional Medical Center • It has now transformed into a community-led initiative to develop a health improvement framework for the County. The framework was adopted on March 2, 2012 at the Stakeholder Summit • CVS builds upon the wellness element of the Countywide Vision championed by Chief Executive Officer, Greg Devereaux

San Bernardino CountyReentry Collaborative (SBCRC) • The Department of Public Health was awarded the Second Chance Act Adult Offender Reentry Planning Grant from the Department of Justice to build on the work of the original Public Health Reentry Task Force • The goal is to develop a strategic plan that will address improved reintegration of the recently incarcerated into our communities and ultimately reduce recidivism rates over the next five years • Partners include: Probation Department, Community Based Organizations (CBOs), Faith Based Organizations (FBOs), Universities, County departments, Local and State Agencies, Local Law enforcement

  8. Federally Qualified Health Centers (FQHC) • Public Health worked with the U.S. Department of Health, and Human Services Resources and Services Administration to designate Hesperia Health Center as a FQHC • The goal is to enhance and expand healthcare services throughout the County, especially in the high desert region

  9. Federally Qualified Health Centers (FQHC) • Hesperia Health Center will provide a place for underinsured and uninsured residents to receive preventative and primary care services • As a FQHC, Hesperia Health Center will have access to federal and state grants and a higher reimbursement rate for services • The grand opening for the Hesperia Health Center was on April 2, 2012
