Safety Committees Reporting accidents and losses: Student injuries: Accident form to be completed following: • Medication error - Copy also goes to Nursing Services • Emergency call - 911 - life threatening situation • Further medical care recommended • Sustains head injury (e.g., blows to the head, whip lash, injuries around eye socket, or any other questionable injury to the head which in the opinion of the Nurse/Health Assistant warrants documentation) • Faulty equipment or unsafe condition existed
