  2. Here are just a few of the great places we will learn about on our trip….. • Smithsonian • The Capital • The White House • Jefferson Memorial • Vietnam Veterans Memorial • The Old Post Office • Arlington Cemetery • The Lincoln Memorial

  3. Mallard Creek Elementary School Behavior Contract It is the goal of our school that all students will be able to attend field trips. In order to ensure the safety of all children away from our campus, students will need to show that they can meet behavior expectations while in school. • If a fifth-grade student is documented in a discipline referral for a major offense from an incident in school, on a school bus, at a school bus stop, or on school grounds, he or she will be placed on a field trip watch-list. The watch-list will be monitored by the fifth-grade teachers as well as Mallard Creek’s administration team.

  4. Once a student is documented in a discipline referral for a major offense for a 2nd time this school year, a parent or guardian will be required to attend a field trip as a chaperone in order for that student to be eligible to attend as well. The parent will be accountable for all chaperone fees for the trip as well. • If a student is documented in a discipline referral for a major offense for a 3rd time this school year, the child will no longer be eligible to attend the Washington, DC trip. Any student and/or chaperone fees that were previously paid for the trip will not be refunded.

  5. Payment Due Dates for the Trip $105 on October 10 $105 on November 7 $105 on December 12

  6. A Word from the PTA • “Attraction Book” Sales are due Friday, September 28th. • Remember that students receive $10 towards their trip for each book sold!

  7. Medical Concerns • A Doctor’s permission form must be obtained prior to the trip • Any prescriptions that your student needs must be labeled with directions and placed in a ziplock bag.

  8. DC Trip Packing List • Pajamas • Underwear/socks • Hairbrush/comb • Toiletries (Toothbrush and toothpaste) • Sunscreen/chap stick • Comfortable Walking Shoes • Daily Outfit • Jacket or Sweater • Poncho (no umbrellas) • Camera, it is student’s responsibility to maintain their own camera. • Cellphone use limited to hotel only, chaperones will monitor all calls, and students cannot have phones with them anytime during the day.
