Vision and outcomes of Integration, cooperation and partnership working • Improve service user experience • Eliminate duplication • Streamline care pathways • Collaborate on early intervention and prevention • Improve safeguarding “The vision is for integrated care and support that is person-centred, tailored to the needs and preferences of those needing care and support, carers and families.”

What does the Act say? Local authorities MUST: Ensure the integration of care and support provision with health provision and health-related provision Co-operatewith each of its relevant partners, and each relevant partner must co-operate with the authority, in the exercise of their functions relating to adults and carers Co-operate where this is needed in the case of specific individuals who have care and support needs
Better Care Closer to Home Strategy (BCCH) • Key joint strategy • Create holistic integrated community services in the community to maintain and improve people’s health and wellbeing • Shift in settings of care from hospital to community • Funding from Better Care Fund will deliver parts of the Strategy
Better Care Fund (BCF) • The BCF is a pooled budget that shifts resources into social care and community health services for the benefit of the NHS and local government • Requirement to measure impact on reducing avoidable hospital admissions and reduce length of stay
Outcome Based Commissioning (OBC) • Initiative of Richmond Clinical Commissioning Group (CCG) and Richmond Council • Aims to transform the delivery of community services based around outcomes that matter most to local people • This model is still in development and some services will be delivered in this way from April 2015.
