The "Care for the Carers Project" in Hong Kong, established in 2008, adopts a case management model to provide holistic support to carers of persons with physical disabilities. With a focus on community-based care and specialized services, the project offers counselling, support groups, and educational resources tailored for caregivers. Over its operation from 2008 to 2012, the initiative demonstrated significant positive impacts on the well-being and satisfaction of both carers and the individuals they care for, although further rigorous evaluation methods are recommended.
