Mini-Grant Application • You will be sent an email from SRP Accounts Payable to set up an account for you in a few days (after I approve) • Follow the directions on the link • You will receive a W-9 form. You need to print it, sign it, and scan it back in and return it. • The information only goes to Accounts Payable and is destroyed once payment is made.
