Jun 2014 Florida Hospital LaboratoryPediatric Vancomycin Trough Normal Rangeand Critical Value Changes Proposed Effective Date: July 1, 2014 Pharmacy requested the pediatric range and critical value match the adult values in order to be consistent with best practice. This is will result in one vancomycin trough normal range and critical value for all age groups. CURRENT Approved Changes • Vanco Trough Range • Pediatric: 5.0 – 15.0 mcg/ml • Vanco Trough Critical Value • Pediatric: = / > 20.0 mcg/ml • Vanco Trough Range • Pediatric: 10.0 – 20.0 mcg/ml • Vanco Trough Critical Value • Pediatric: = / > 25.0 mcg/ml THERE IS NO CHANGE TO THE ADULT VALUES *** Specimen Requirement Change for BOTH Adult & Pediatric Patients *** CurrentNew Green Top Tube Yellowor Red Top Tube MEC Approval: 3/24/14
