  3. MATERIAL I METODE • estudi longitudinal, prospectiu, descriptiu d’intervenció. • 8 mesos, dies laborables. • Població: pacients ingressats al SUH durant > 24 h. • Mostra d’estudi: > temps que havien ingressat al SU.

  6. OBJECTIU 1 RELLEVÀNCIA CLÍNICA • 4 CLÍNICS independents. • Valoració 1-6. • Grau de concordança. Intervencions acceptades -PRM

  8. OBJECTIU 2 ESTALVI COSTOS • Costos directes d’adquisició de la medicació. • Diferència de costos diaris de la medicació abans i després de fer-se efectiva la proposta de canvi X dies d’estada SU.

  10. POBLACIÓ • N = 922 pacients inclosos Homes 542 58,8 % Dones 380 41,2 % Edat 17 – 99 anys (mitjana 73,4 anys). • N = 400 pacients intervinguts 43,4 %

  19. CONCLUSIONS (i) • Un programa assistencial d’atenció farmacèutica en els SUH, es considerada pels clínics com una millora en l’atenció dels seus pacients, produint un estalvi en costos directes.
