  3. PA SPREAD has been approved to award…… • ABIM Part IV MOC Credits • ABFM Part IV MOC Credits • 20 CME Credits Through AAFP AAFP CME Credits can be claimed by physicians and nurses!

  4. Approval Processes 20 Prescribed American Academy of Family Physician Credits • CME credits restricted to those that have attended at least 3 of 4 Learning Sessions, participated in monthly webinars, who have been active in planning, implementing, monitoring PDSAs and clinical performance improvement. • PA SPREAD credits count towards Patient Safety requirement!

  5. Approval Processes Part IV Maintenance of Certification (MOC) • Restricted to physicians who are engaged in 12months of quality improvement via PA SPREAD, participated in monthly webinars, active in planning, implementing, monitoring PDSAs and clinical performance improvement. • Must have attended 3 of 4 Learning sessions.

  6. Did you not meet the 3 of 4 Learning Session attendance requirement? • If you attended 2 Learning Sessions and met all other requirements specified, please pick up one of our CME/MOC Attestation Forms. • PA SPREAD faculty and practice facilitators will evaluate and notify you if your request has been approved!

  7. If you have any issues or questions about MOC or CME, please let us know! Those that automatically qualified for credit, will receive their certificates after the conclusion of the Learning Session!
